SUBJECT : Spot Report, re – Reckless Imprudence Resulting to

Homicide

1. On or about 9:00Am of February 12, 2024 a vehicular incident transpired along the National
Highway located at Zone 5, Brgy. Lourdes, Buhi, Camarines Sur. Initial investigation was conducted by the
elements of this station revealed that on above-mentioned time, place and date, while Mitsubishi Delica
white and black combination bearing plate no. 746YFC driven by one, FRANK JAMES IMPERIAL y Erodico,
20 years old, single, student and a residence of Brgy. Tambo, Buhi Camarines Sur was recklessly
traversing the road of Brgy. Lourdes in this municipality, hit the rear bumper of padyak, color yellow and
green bearing plate no. 304, driven by DAVE BELLES y Ollinares 36 years old, married, padyak driver and
a residence of Zone-3, Brgy. Lourdes, Buhi, Camarines Sur.

2. The driver of the padyak sustained fatal injuries at different parts of body and were rushed to a
hospital in Iriga City for medical treatment but was pronounced dead in arrival (DOA) by the attending
physician thereat.

3. The suspect was arrested after the incident and is now under the custody of this Police Office for
investigation and proper disposition.

4 .Necessary documents are being prepared for filing of appropriate charges against the suspect.

5. The Motive behind the incident is still unknown.

6. Progress report will follow.
