1. Reference

2. This pertains to the Vehicular incident transpired along the National Highway
located at Zone 5, Brgy. Lourdes, Buhi, Camarines Sur, wherein a Mitsubishi
Delica white and black combination bearing plate no. 746YFC driven by one,
FRANK JAMES IMPERIAL y Erodico, 20 years old, single, student and a
residence of Brgy. Tambo, Buhi Camarines Sur was hit the rear bumper of
padyak, color yellow and green bearing plate no. 304, driven by DAVE
BELLES y Ollinares 36 years old, married, padyak driver and a residence of
Zone-3, Brgy. Lourdes, Buhi, Camarines Sur.

3. FRANK JAMES IMPERIAL y Erodico admitted to have been drunk while


driving after being questioned in the Buhi Municipal Police Station at on or
about 10:00AM of February 12, 2024

4. The driver of the padyak sustained fatal injuries at different parts of body and
were rushed to a hospital in Iriga City for medical treatment but was
pronounced dead in arrival (DOA) by the attending physician thereat.

5. According to the results of the autopsy examination conducted by Doctor


Nekki Bongala of Iriga Hospital, the cause of death is the protruding broken
ribs that damage the heart which caused further damaged to the artery that
caused exsanguination or extreme blood loss. The manner of death is
confirmed to be a caused by too much external force of the suspect vehicle.

6. Arrested suspects, FRANK JAMES IMPERIAL y Erodico 20 years old, single,


student and a residence of Brgy. Tambo, Buhi Camarines Sur had undergone
inquest proceedings conducted by PMS. LORD IBN FRANCOIS S.
BONGALA at Prosecutor's Office, Naga City and were filed with the crime of
RECKLESS IMPRUDENCE RESULTING TO HOMICIDE CASE NO. 1214-
2021.
