DRILL NOTIFICATION

Type of drill : Chemical exposed & Medivac Location of Drill : Warehouse KOIN
Date of drill : Tuesday, 27 April 2021 Time Required : 1.5 Hour (10:00 – 11.30)

1. Objective
- To find out capabilities of ERT involved and emergency communication system
(emergency organizational)
- To know the response of HDEC ERT, Area ERT/ Fire warden, Area Supervisor and
environment team.
- To familiarize with Chemical exposed and medical evacuation,
- The site workers be familiar to respond and actively move to muster point assembly and keep away
from chemical spillage at the time of incident.
- To find out the exact time is needed to evacuate the IP and clean up the chemical spillage.
2. Scenario
Around 9.30 AM There is a chemical (EPOXY) transfer activity using forklift from warehouse to WWTP
Area. During transfer to temporary storage area of WWTP accidentally the operator does not see any
wooden block then hit it that makes the forklift unbalance at front of KOIN Security office, causing the drum
drop from forklift and spills the chemical on the ground due to contact with solid material resulting the
chemical splash into eyes and body of worker who walking around it. In scene witness immediately ask for
help and inform to Koin safety man the detail incident. Koin Safety man and site supervisor go to the
incident location. Safety man inform to KOIN ERT commander and KOIN first aider. KOIN emergency
response team bring the stretcher to evacuate the victims to get the nearest emergency eye wash.

Safety personnel install barricade at the incident and spillage area. KOIN ERT commander then call HDEC
ERT commander then calls ambulance to mobilise an ambulance and send IP to HDEC Clinic to get further
treatment. KOIN ERT commander also inform to HDEC Environment team regarding the chemical spillage
to get correct method of cleaning up the spill that must be carried by team using sand.
