and Subcontractors who directs, supervise and control
hazardous activities where a permit to work is necessary.

HSJV Construction (PTW) Permit to Work Categories & work tasks are classified
into the following categories:
• Working at height
• Electrical Work
• Scaffolding
• Pressure testing
Mechanical excavations
Manual excavations of depth exceeding (30 Cm)
Schematic Diagrams of underground utilities.
When an excavation depth is greater than 1.2 meters it then becomes a confined space. Therefore a Confined space Permit is
then required along with Gas testing by an authorized gas tester (AGT)
Radiography testing in BNPP is conducted by JisCorp (3rd Party) and under the supervision of
KEPCO.
The Permit Issuer may allow certain specific, routine, non-hazardous tasks to be performed
in his/her area without the issue of a permit to work.
The Permit Issuer must satisfy himself that the risks are as low as reasonably practicable
(ALARP) and that the tasks are performed by suitably competent personnel.
Many of the tasks that do not require a permit or the use of a formal procedure are
themselves the subject of written procedures that have been, or will be approved method
statement and risk assessment. Such activities include, but are not limited to, the following:

Other Documents are not Permits to Work and they should never be used alone authorize work to proceed.
Method Statements, SA must be prepared for all work which requires a Permit to Work.
Prior to undertaking any non-routine, high risk or hazardous construction activities, a Work
Method Statement will be submitted for approval to HSJV and BNPP Project which will
include the risk assessment and Work Method Statement (WMS) The risk assessment

Section 1 Activity
Identify the exact location and briefly describe the activity that is to be performed
Include the time, date applied , name of the persons.
Gloves
The Permit Holder shall ensure the work site is left in a clean, safe state after the completion of work
and where required a process is in place to manage any follow-up work; The Permit Holder shall sign the
last section of the Permit to Work form; and Permit Issuer shall initiate the removal of hazard controls
originally installed for the work to take place.
When the work environment has been returned to a state of readiness for return to normal duties, the
Permit Issuer shall confirm and sign the ‘permit closure’ section of the Permit to Work form on both
original and duplicate copy.
Yellow
• The Third copy will be recorded and filed by
the Permit to Work Controller.
The assigned Competent Person Permit Issuer/ Permit Holder will be required
to conduct regular inspections of the work area and assess to ensure that the
conditions remain unchanged and remain safe for work to continue.
The Permit Holder must ensure the implementation of safe working practices
and the monitoring of his work area for any change in conditions.
Should there be any changes in work conditions that potentially effect health
and safety, all work will be suspended immediately and the PTW withdrawn.
A new PTW must be considered taking into account the changed conditions
before recommencing the work.
The Permit Holder / Authorized Permit Issuer will be authorized to stop any
work whenever he thinks fit, or when the stated precautions in the PTW are
not being fully observed and implemented.
The Permit Holder is responsible for the safe execution of the work and will
be present onsite at all times during the execution of the work.
If the Permit Holder is absent whilst the work is being carried out, he will
appoint a responsible supervisor to oversee the operation until his return.
Such hand over will be properly recorded.
o Both outgoing and incoming Permit Holder and the Permit Issuer will sign of the PTW.
o The incoming Permit holder will take over full responsibility from the outgoing Permit holder for the
work being performed.
No work will be allowed to start until the Authorized Person of the Permit Issuer and
Permit Holder have performed a pre-work inspection of the work area, preferably with
the attendance of the area HSJV Safety Supervisor. If, following this inspection, the
Permit Issuer is satisfied that the required control measures have been implemented he
will authorize the work to proceed by signing Section 3 of the Permit.
• When the work is completed, the Permit Holder will inform the
Authorized Permit Issuer, who will inspect the work site (jointly with
the Permit Holder). If the Permit Holder and Authorized Permit Issuer
are satisfied that all work (including clean up) is complete, both will
sign Section 6 of the PTW.
• When the work has been completed, all 3 copies of the PTW will be
collected by the Authorized Permit Issuer. All copies of the completed
PTW and any associated Supplementary Certificates will be filed by the
PTW Controller for the purposes of future audit and investigation
purposes. Both PTW and Supplementary Certificates will be kept for a
period of at least 12 months (to facilitate auditing) following completion
of work.
The PTW will be valid for a maximum of 7 days, but must be renewed / revalidated daily or at
the shift change of the Permit Holder and Authorized Person Permit Issuer . A PTW can be
re-validated only with the following provisions:
- No change has occurred, or is anticipated at the work site, that may prove hazardous
to, or incompatible with, the work specified in the PTW.
- The Permit Issuer, Permit Holder have completed their inspection of the work area
and are satisfied with the conditions, as well as the safe execution of the work stated
in the PTW.

The Authorized Person (Permit Issuer) shall have the authority to issue and sign permits on
behalf of the entity. It is essential that the AP has sufficient knowledge about the hazards
associated with the relevant PTW and successfully passed the PTW course, to allow them to
identify those hazards and control measures (e.g. isolations) correctly. Including:
• Shall ensure that all foreseeable hazards associated with the proposed job have been
identified and suitably assessed;
• Shall ensure that all steps necessary to ensure the safety of the site or installation have
been identified;
• Shall ensure the work site has been examined, and all precautions specified to be taken
before work commences (including isolations) have in fact taken and shall remain effective
while the permit remains in force;
• Shall verify that the permit holder, including further staff tasked with undertaking works
under the PTW, have sufficient competence to undertake the role and fully understand the
risks.
• Shall ensure that the permit holder is aware of the precautions taken, any additional
ones which are to be taken, particular equipment to be used or worn, and any other
procedures which are to be followed;
• Shall ensure that any work activities that may work activities that may conflict with
one another are clearly identified and conflictions are avoided or precautions included
on the permit (e.g. use of welding shields);
• Shall ensure that all relevant people are aware of the permit’s duration, and action to
be taken if the work is suspended;
• Shall authorizing, by signature, the work to proceed following confirmation that all
controls are in place and have been signed off;
• Shall ensure that copies of all issued permits are displayed at an appropriate location
and in a consistent arrangement so that site personnel can readily see and check which
equipment is under maintenance and not available for operation;
• Shall undertake ongoing inspections of the PTW to ensure risk control measures are
being maintained;
• Shall ensure that the work site is examined at any time when work is suspended
and before it is restarted, and finally when the work is completed to ensure that it is
in a safe condition;
• Shall ensure that the shift handover procedure is properly followed; and
• Shall ensure that the worksite is examined on completion of the works and the
• PTW is correctly closed and the relevant persons informed ensuring that instructed
persons working under the terms and conditions of the PTW.
• Understand the extent of the location/equipment/work site covered by the PTW
• Understand the extent of the hazards/danger points present and agreed controls
•Shall ensure that they and the people working with them understand the operation of
(and the consequences of non-compliance with) the permit-to-work systems applicable to
the areas in which they are responsible for work;
•Shall ensure any necessary information, instruction or training is given to users to ensure
that they understand the permit-to-work systems and the specific precautions required for
their work;
•Shall ensure that the work party and permit users fully understand their responsibilities
under the permit-to-work system;
•Shall be available and contactable on site during the work duration;
•Shall ensure that the conditions and precautions specified in the permits are fully
understood, implemented and effectively monitored;
•Shall immediately stop / suspend work if conditions require or if the PTW requires change;
•Shall communicate effectively with all parties during the operation of the Permit; and;
•Shall ensure that all parties are aware of the completion of the works and the permit is
correctly closed with the Permit Issuer. Check work area is safe prior to close out

Roles & Responsibilities (Cont.)
Note:
The same person shall not have the role of both Permit Issuer Authority and Permit
Holder Party for any PTW. (i.e. You cannot issue a Permit to Yourself)
HSJV and its sub-contractor shall authorize and nominate qualified individuals for
the work to be performed under HSJV Permit to Work.
Site Manager
Permit Controller Manager/Coordinator: Personnel who have passed the PTW training
course. He / she is the person in charge to check relevant permits and associated
supplementary documents. He/she is responsible for keeping the records of all PTW’s and the
related SD’s in the PTW Register.
He / She shall also be responsible to ensure the following:
• Ensure that work activities requiring PTW are planned and coordinated in
order to avoid caused by simultaneous activities.
• Monitors the process and the effectiveness of PTW system throughout the site
and maintaining the records in the permit registry.
• Ensure that appropriate work procedures are established and maintained for
all work under PTW
• The permit and certificates are correctly and accurately completed in
accordance with the PTW procedure.
• All Permits and certificates are recorded at the time of issue / close out and
maintained in the PTW Register hard and electronic copies.
• Securely store completed and cancelled PTW’s following cancellation for the
agreed period of 1 year.
The Permit Control Manager is the prime authority of the construction project in controlling
of all the issuance of work permits and is ultimately responsible for HSE. The Permit Control
Manager shall ensure that a competent Authorized Person is appointed to issue and manage
Permits, including :
- Permit Controller Manager shall be a designated personnel appointed by HSJV Site
Manager with full authority to ensure that an appropriate permit-to-work system is
operated;
- Shall ensure that appropriate work procedures are established and maintained for all work
done under the permit to work system;
- Shall ensure that arrangements are made for the workforce to be made aware of the
permits and systems, and trained in their operation;
- Shall ensure that the permit to work system is monitored to ensure that it is effective and
correctly applied;
- Shall ensure appropriate procedures are in place for any emergency situations that may
arise following of non conformity with the PTW; resources to enable the permit-to-work
system to be properly implemented.
Fire Watchers : Personnel appointed as Fire Watchers shall be adequately trained and
shall monitor work areas, and the precautionary measures taken in those areas, when
specified by the Permit Issuer on the PTW. Fire Watchers shall undertake fire response
activities as required.
• Wear an orange vest and has a portable air horn, arm band and helmet sticker.
• Assure that all required fire fighting equipment is available in the working
area prior to starting the job.
• Assure that all permit conditions are maintained.
Confined Space Attendant : A Confined Space Attendant will have received adequate
training to allow him to carry out his duties responsibly. At all times during a confined space
entry operation, he must remain outside the confined space, in a safe atmosphere, and
perform the assigned duties.
Safety Supervisors: are to assist the HSJV Site HSE Manager in implementing, updating
and maintaining the PTW procedure.

Workers Responsibilities
All entities shall ensure that all relevant parties are competent to undertake their role
within the Permit to Work Procedure.
Trainings course for Permit to work system is only applicable to the Supervisory level,
each disciplined engineers by department (e.g Electrical engineer, Mechanical engineer,
Piping engineer, Civil engineer.) & Foreman.
