Part one: Reading Comprehension

Directions: Read the following passages carefully. Each one is followed by
several questions about it. Choose the one best answer, (a), (b), (c), or (d), to each
question. Then on your answer sheet, fill in the space that corresponds to the
letter of the answer you have chosen. Base your answer to each question on the
information given in the passage only.
PASSAGE ONE
It is not hard to understand why the principle of confidentiality is important
to both doctor and patient. The doctor must depend on the frankness and
openness of the patient in order to make an accurate diagnosis. This often
requires that the patient reveal embarrassing facts about himself. In order to
alleviate anxiety about such facts being revealed, the doctor promises not to
violate any confidences. Obviously, if patients thought that doctors routinely
publicized their problems, many would be extremely evasive in what they
revealed to the doctor; some would even forego consulting a doctor. So, while
ethical duty to the patient might play a role in the doctor's observance of the
principle of confidentiality, there is a decisive practical reason for it as well. It
enables him to discharge more effectively his professional responsibility to
diagnose and treat the patient.
1. The paragraph views the patient's frankness as a achieving proper
diagnosis.
a. preparation in

c. performance for

b. requisite for

d. barrier against

2. The word "This" (line 3) refers to .. .

a. revelation of the diagnosis
b. patient's frankness and openness
c. principle of confidentiality
d. doctor's frankness
3. The text recommends doctors to .. the principle of confidentiality.
a. stop being worried about
b. direct patient consultations at
c. treat his patients regardless of
64

www.phse.ir

d. remain loyal to
4. The physician's release of confidential records.. .
a. could seriously harm patient confidence
b. is routinely carried out nowadays
c. is recommended on medical grounds
d. could hasten patient recovery
5. Patients' evading openness and foregoing consultation are referred to as a
likely .. .
a. response to the release of their confidential records
b. tactic to seek patients' cooperation
c. sequence of doctor-patient consultations
d. way of observing the principles of confidentiality
PASSAGE TWO
The mind-body effect should not be surprising in view of the experience over
the years with placebos. The term placebo is used to describe a "pill" that
contains no medical ingredients but that often produces the same effect as
genuine medication. Placebos provide ample proof that expectations can have an
effect on body chemistry.
According to a recent article on placebos in Medical World News, studies
conducted over the past 25 years have shown that placebos satisfactorily relieved
symptoms in an average of 35 percent of patients tested. These symptoms
include: fever, severe postoperative pain, anginal pain, headache, and anxiety,
among other complaints. The explanation for this strange phenomenon is that
the human mind can create actual changes in body chemistry as a result of what
it believes. If, for example, a person believes that a certain medication contains a
substance that can accomplish a specific need, the body tends to move in that
direction.
6. It is implied that .. .
a. placebos are more effective than true medicine in most cases.
b. the relationship between the body and mind has not been established well
c. placebos must be prescribed to avoid side-effects of the drugs
d. the human physical and mental aspects are closely related
7. Experiments with placebos have . .
a. indicated the interaction between body and mind
65

www.phse.ir

b. increased the expectations one can have from his body chemistry
c. rejected the great effects of real medication on the body
d. revealed the priority of mind over body in the treatment of diseases
8. According to this reading selection, the body chemistry . .
a. is rarely affected by one's belief in what the drug does
b. may undergo change when one thinks it does
c. affects people's attitudes towards drugs and placebos
d. produces necessary changes in the drug taken
9. It is implied that the effect of a real drug .
a. is independent of people's attitudes toward it
b. depends on how its ingredients affect the chemistry of the body
c. may be accelerated or otherwise based on one's belief
d. depends on the time and type of substance which is mixed
PASSAGE THREE
Breathing secondhand smoke can be harmful to children's health including
asthma, Sudden Infant Death Syndrome (SIDS), bronchitis and pneumonia and
ear infections. Children's exposure to secondhand smoke is responsible for (1)
increase in the number of asthma attacks and severity of symptoms in 200,000 to
1 million children with asthma; (2) between 150,000 and 300,000 lower
respiratory track infection (for children under 18 months of age); and (3)
respiratory tract infections resulting in 7,500 to 15,000 hospitalizations each
year. The developing lungs of young children are severely affected by exposures
to secondhand smoke for several reasons including that children are still
developing physically, have higher breathing rates than adults, and have little
control over their indoor environments. Children receiving high doses of
secondhand smoke, such as those with smoking mothers, run the greatest risk of
damaging health effects. These mothers are not to smoke in their home and car
until they can quit, nor are they to allow family and visitors to do so.
10. Asthmatic attack is referred to as one of the .. exposure to secondhand
smoke.
a. responsibilities

c. severities

b. complications

d. deficiencies

11. In the passage, we read that ear infections in children may be .

a. the result of their living habits
66

www.phse.ir

b. a severe threat to the child's life

c. due to exposure to secondhand smoke
d. treated by exposing ear to cigarette smoke
122. One reason that adults are less susceptible to smoke than babies with
smoking mothers is their .. .
a. higher capacity

c. developing lungs

b. breathing rates

d. indoor environment

13. Babies and toddlers are especially vulnerable to the health risks from
secondhand smoke as they .. .
a. are still being breast-fed
b. start smoking at very young age
c. can hardly change the environment
d. are independent of their surroundings
14. Smoking mothers are recommended to .
a. quit their children

c. stop smoking immediately

b. move to another room

d. stop smoking indoors

15. A good title for this passage is .. .

a. child Risks
b. Smoking Mothers
c. Smoking Rooms
d. Child Susceptibility to Smoke Effects
PASSAGE FOUR
The passage of an electric current through the body may be without effect,
may cause sudden death, or may cause thermal injury to organs interposed in
the pathway of the current. Many variables are involved, but the most important
are the resistance of the tissues to the conductance of the electric current and the
intensity of the current. The greater the resistance of tissues, the greater the heat
generated. Although all tissues of the body are conductor of electricity, their
resistance to flow varies inversely with their water content.
The thermal effects of the passage of the electric current depend on its
intensity. High-intensity current, such as lightning coursing along the skin,
produces linear arborizing burns known as lightning marks. Sometimes intense
current is conducted around the victim 9so-called flashover), blasting and
disrupting the clothing but doing little injury. When lightning is transmitted
67

www.phse.ir

internally, focal hemorrhages from rupture of small vessels may be seen in the
brain. Sometimes, death is preceded by violent convulsions related to brain
damage.
16. It is inferred from the first paragraph that tissues with less water . those
with more water.
a. are more resistant than
b. have the same resistance as
c. are less resistant than
d. have double resistance as
17. The amount of heat which is produced mostly depends on the .
a. resistance of the tissues

c. types of one's clothing

b. lightning marks

d. arborizing burns

18. It is inferred from paragraph one that tissues which are more resistant to
conductivity .. .
a. contain large amount of water
b. are more prone to injury
c. can intensify the current
d. diminish much of the heat
19- As to lightning coursing along the skin and flashover, . .
a. the former is usually less damaging than the latter
b. the latter passes rapidly along the skin
c. both operate in the same manner
d. none is caused by low-intensity currents.
20. According to the second paragraph .. .
a. death may be followed by brain damage
b. death may follow violent convulsions
c. violent convulsions can proceed from death
d. brain damage can proceed from death
PASSEG FIVE
most community wastewater treatment plants are not designed to treat
harmful substances such as paints, solvents, oil, or pesticides, so these chemicals
should not be flushed down the toilet or dumped into the sewer. Similarly, most
community landfills are not equipped to handle hazardous materials; these
substances should not go in the trash or be dumped on the ground or buried.
68

www.phse.ir

Do use and dispose of harmful materials properly. More importantly, limit

use of such products and substitute a no hazardous product when possible. When
this not possible, buy only as much as you need. Hazardous products used
around the home include some oven cleaners, toilet bowl cleaners, drain cleaners,
bleaches, rust removers, paints, solvents, polishers, carpet and furniture cleaners
and glues. If you don't completely use up a hazardous product, give it to someone
who needs it or take it to a hazardous waste or recycling center. Take advantage
of household hazardous waste collection days if your community offers them.
If you must use pesticides and fertilizers on your lawn and garden, use them
in moderation. Your county extension service can provide information on
natural ways to control lawn, garden, and tree pests which can reduce on
chemicals.
21. Paints, pesticides, and bleaches are examples of .
a. non-hazardous products
b. substances often buried in landfills
c. chemicals dumped into the sewer
d. materials harming the environment
22. A hazardous product not utilized completely is recommended to be . .
a. given away

c. done with temporarily

b. thrown away

d. taken to community landfills

23. Proper use and disposal of harmful material discussed in the passage is a
. .
a. recommended precaution

c. hazardous action

b. curative measure

d. common practice

24. The use of pesticides is recommended . .

a. in abundance

c. along with fertilizers

b. as the last resort

d. as a most effective means

25. This is an extract of a text on .

a. waste disposal sufficiency
b. harmful material developments
c. wastewater treatment
d. protecting environment

part one: Reading Comprehension
Direction: Read the following passages carefully. Each one is followed by several
question about it. Choose the one best answer, (a), (b), or (d), to each question.
Then on your answer sheet, fill in the space that corresponds to the letter of the
answer you have chosen. Base your answer to each question on the information
given in the passage only.
PASSAGE ONE
It is not hard to understand why the principle of confidentiality is important to
both doctor and patient. The doctor must depend on the frankness and openness
of the patient in order to make an accurate diagnosis. This often requires that
the patient reveal embarrassing facts about himself. In order to alleviate anxiety
about such facts being revealed, the doctor promises not to violate any
confidences. Obviously if patients thought that doctors routinely publicized their
problems, many would be extremely evasive in what they revealed to the doctor;
some would even forego consulting a doctor. So, while ethical duty to the patient
might play a role in the doctor's observance of the principle of confidentiality,
there is a decisive practical reason for it as well. It enables him to discharge more
effectively his professional responsibility to diagnose and treat the patient.
1- The paragraph views the patient's frankness as a . achieving proper
diagnosis.
a. preparation in

c. performance for

b. requisite for

d. barrier against

2- The word "This" (line 3) refers to . .

a. revelation of the diagnosis
b. patient's frankness and openness

a. stop being worried about

c. treat his patients regardless of

b. direct patient consultations at

d. remain loyal to

4- The physician's release of confidential records.. .

a. could seriously harm patient confidence
b. is routinely carried out nowadays
c. is recommended on medical grounds
d. could hasten patient recovery
5- Patients' evading openness and foregoing consultation are referred to as a
likely .
a. response to the release of their confidential records
b. tactic to seek patients' cooperation
c. sequence of doctor patient consultations
d. way of observing the principles of confidentiality

PASSAGE TWO
The mind body effect should not be surprising in view of the experience over
the years with placebos.

74

symptoms in a average of 35 percent of patients tested. These symptoms include:

Fever, severe postoperative pain, pain, headache, and anxiety, among other
complaints. The explanation for this strange phenomenon is that the human
mind can create actual changes in body chemistry as a result of what it believes.
If, for example, a person believes that a certain medication contains a substance
that can accomplish a specific need, the body tends to move in that direction.
6- It is implied that .. .
a. placebos are more effective than true medicine in most cases
b. the relationship between the body and mind has not been established well
c. placebos must be prescribed to avoid side effects of the drugs
d. the human physical and mental aspects are closely related
7- Experiments with placebos have .
a. indicated the interaction between body and mind
b. increased the expectations one can have from his body chemistry
c. rejected the great effects of real medication on the body
d. revealed the priority of mind over body in the treatment of diseases
8- According to this reading selection, the body chemistry .
a. is rarely affected by one's belief in what the drug does
b. may undergo change when one thinks it does
c. affects people's attitudes towards drugs and placebos
d. produces necessary changes in the drug taken
9 It is implied that the effect of a real drug . .
a. is independent of people's attitudes toward it
b. depends on how its ingredients affect the chemistry of the body
c. may be accelerated or otherwise based on one's belief

75

d. depends on the time and type of substance which is mixed

PASSAGE THREE
Breathing secondhand smoke can be harmful to children's health including
asthma, Sudden Infant Death Syndrome (SIDS), bronchitis and pneumonia and
ear infections. Children's exposure to secondhand smoke is responsible for (1)
increase in the number of asthma attacks and severity of symptoms in 200,000 to
1 million children with asthma, (2) between 150,000 and 300,000 lower
respiratory tract infection (for children under 18 months of age); and, (3)
respiratory tract infections resulting in 7,500 to 15,000 hospitalizations each
year. the developing lungs of young children are severely affected by exposures
to secondhand smoke for several reasons including that children are still
developing physically, have higher breathing rates than adults, and have little
control over their indoor environments. Children receiving high doses of
secondhand smoke, such as those with smoking mothers, run the greatest risk of
damaging health effects. These mothers are not to smoke in their home and car
until they can quit, nor are they to allow family and visitors to do so.
10- Asthmatic attack is referred to as one of the .. of exposure to
secondhand smoke.
a. responsibilities

c. severities

b. complications

d. deficiencies

11- In the passage, we read that ear infections in children may be .

a. the result of their living habits
b. a severe threat to the child's life

76

c. can hardly change the environment
d. are independent of their surroundings
14- Smoking mothers are recommended to .. .
a. quit their children

c. stop smoking immediately

b. move to another room

d. stop smoking indoors

15- A good title for this passage is .

a. Child Risks

c. Smoking Rooms

b. Smoking Mothers

d. Child Susceptibility

PASSAGE FOUR
The passage of an electric current through the body may be without effect, may
cause sudden death, or may cause thermal injury to organs interposed in the
pathway of the current. Many variables are involved, but the most important are
the resistance of the tissues to the conductance of the electric current and the
intensity of the current. The greater the resistance of tissues, the greater the heat

77

www.phse.ir

generated. Although all tissues of the body are conductor of electricity, their
resistance to flow varies inversely with their with their water content.
The thermal effects of the passage of the electric current depend on its intensity.
High-intensity current, such as lightning coursing along the skin, produces linear
arborizing burns known as lightning marks. Sometimes intense current is
conducted around the victim (so-called flashover), blasting and disrupting the
clothing but doing little injury. When lightning is transmitted internally, focal
hemorrhages from rupture of small vessels may be seen in the brain. Sometimes,
death is preceded by violent convulsions related to brain damage.
16- It is inferred from the first paragraph that tissues with less water those
with more water.
a. are more resistant than

c. are less resistant than

b. have the same resistance as

d. have double resistance as

17- The amount of heat which is produced mostly depends on the .

a. resistance of the tissues

c. types of one's clothing

b. lightning marks

d. arbor zing burns

18- It is inferred from paragraph one that tissues which are more resistant to
conductivity .
a. contain large amount of water
b. are more prone to injury
c. can intensify the current
d. diminish much of the heat
19- As to lightning coursing along the skin and flashover,
a. the former is usually less damaging than the latter
b. the latter passes rapidly along the skin

78

www.phse.ir

c. both operate in the same manner

d. none is caused by low-intensity
20- According to the second paragraph .
a. death may be followed by brain damage
b. death may follow violent convulsions
c. violent convulsions can proceed from death
d. brain damage can proceed from death
PASSAGE FIVE
Most community wastewater treatment plants are not designed to treat harmful
substances such as paints, solvents, oil, or pesticides, so these chemicals should
not be flushed down the toilet or dumped into the sewer. Similarly, most
community landfills are not equipped to handle hazardous materials; these
substances should not go in the trash or be dumped on the ground or buried.
Do use and dispose of harmful materials properly. More importantly, limit use of
such products and substitute a no hazardous product when possible. When this is
not possible, buy only as much as you need. Hazardous products used around the
home include some oven cleaners, toilet bowl cleaners, drain cleaners, bleaches,
rust removers, paints, solvents, polishers, carpet and furniture cleaners and
glues. If you don't completely use up a hazardous product, give it to someone
who needs it or take it to a hazardous waste or recycling center. Take advantage
of household hazardous waste collection days if your community offers them.
If you must use pesticides and fertilizers on your lawn and garden, use them in
moderation. Your county extension service can provide information on natural
ways to control lawn, garden, and tree pests which can reduce reliance on
chemicals.
