MOCK DRILL

DATE: 19/02/2017
DIU: Diesel Leak at 71FCV1801 D/S Flange (71D20 Outlet)

OBJECTIVE: To gauge the response of operating crew and assess their preparedness in
handling process related emergencies such hot diesel leak from a control valve D/S flange.

SCENARIO: A mock drill is conducted at DIU unit at 10:50 hrs. To 11:20 hrs. On 19/02/2017
by announcing leak at D/S flange of FCV 1801 on 71D20 outlet.

MANPOWER:

DCS Officer : SS/MRS


Field Officer : RV
Field Crew : DIU:RKD/AHR, HGU:VKN/PKP, SRU:SWK/JSP

SEQUENCE OF EVENT:

POSSIBLE COURSE OF ACTION:

1. The flow through 71FCV1801 is to be reduced by DCS Officer.


2. The bypass valve is to be opened.
3. The FCV1801 is to be isolated by shutting the U/S and D/S block valves.
4. Fire water should be started and a hose is to be deputed to wash away the hot diesel.

WHAT COULD GO WRONG:

1. The bypass valve or block valves may be jammed.


2. The hot diesel jet from the leak may prevent to open bypass and isolate block valves.

OBSERVATIONS:

1. Overall crew performance was found to be good during this mock drill and everybody
exhibited the basic operational skill-set to respond positively to an actual emergency case.
2. PA system worked satisfactorily.(at a distance of 15 meters from the leak spot)
3. Approach to handle the block valves and bypass valves is provided.
4. The block valves are in serviced condition.
5. Fire water system found to be in working condition.

SUGGESTIONS:

1. While communicating to the Field technicians, the DCS officer should tell the general
location apart from the specific equipment/ position which has malfunctioned. For ex,
while communicating a leak on flange of 71FCV1801, generalized location such as D20
bottom must be told.
2. An emergency fire water hose should be kept in OPFR1, so that in case of need it could
be deployed without waiting for Fire and Safety personnel to reach the emergency
location.
