I. Pre-Planning

The teachers together with RODEL G. BEJARIN had their meeting for the culminating
activities of Nutrition Month 2019. The activities to be done were the following:

A. Parade of the pupils with baskets of fruits and vegetables


B. Program with presentation of Mr.& Ms. Nutrition Month and special number of each garde
level.
C. Cooking Festival of parents together with their children.
D. Poster making and Slogan Contest about the Theme of the Nutrition Month.
E. Lunch time of pupils.
II. Implementation

The activities planned were all accomplished.

III. Evaluation

All activities done were great and accomplished smoothly.

IV. Feedback

The pupils enjoyed the celebration and gained knowledge about the importance of good
nutrition to their health.

Minutes of the First Homeroom PTA Meeting (June 26, 2016) The first General PTA
Meeting for SY. 2019-2020 was held on June 6.2019 at the schools’ gymnasium. There were 56 parents
who attended the Meeting. It started at 8:30 in the morning. The first part of the meeting was formally
started with a prayer led by Mrs. Mannielle Mae T. Oliva. It was followed by the singing of the Philippine
National Anthem conducted by Mrs. Leny M. Hernandez – grade one adviser. The welcome address was
then delivered by the grade four adviser. At 9:00 in the morning, the meeting proper commenced. The
first agendum of the meeting was the election of the homeroom PTA Officials. The result of the meeting
was the election and recognition of the new set of homeroom PTA Officials, namely: President : Clarito
Costelo Vice-President : Eduardo Evangelista Secretary : Delia Estrella Treasurer : Mariwani Belacsi
Auditor : Gloria Bughao P.I.O : Maylen Pepito : Evelyn Sorima Sgt. at Arms : Mario Ecot : Carmelita
Garing Muse : Malou Pahulayan During the meeting, the matter about the pupils’ concern, particularly
their behavior were brought to attention. It was agreed that the parents would follow up on their
children’s attitude on attendance and class interaction. Furthermore, as per the agendum of the school’s
development and maintenance, it was communicated to the parents that the local community and the
school will coordinate with the beautification and cleanliness of the classrooms and school
surroundings. The parents were assigned locations which they will be held responsible for. Details have
yet to be agreed upon. As all other pertinent concerns were dealt with for the time being, the meeting
was adjourned at exactly 10:30 in the morning, with the same number of attendees. Prepared by: Delia
