JOINT AFFIDAVIT TO TWO DISINTERESTED PERSONS

We, NATHANIEL Q. ACEBO and MA ELENA B. CIRERA, both

Filipinos, of legal age, and residents of Brgy. Minuhang, Barugo, Leyte,
respectively, after having been duly sworn in accordance with law, hereby
depose and state:

1. That we both personally know the person of ERNESTO A.

DACARA, a resident of Brgy. Guindaohan, Barugo, Leyte;

2. That we also know and we hereby declare that said ERNESTO A.

DACARA was born on July 29, 1966 at Brgy. Guindaohan, Barugo,
Leyte to Spouses JOSE B. DACARA and MARIA A. DACARA;

3. That we both know the fact that ERNESTO A. DACARA is a

member of Social Security System (SSS);

4. That per record of SSS, his birthdate was erroneously recorded as

“JULY 16, 1966” instead of “JULY 29, 1966”;

5. That a copy of his Certificate of Live Birth issued by the Philippine

Statistics Authority (PSA), shows the correct record of his birthdate
which is JULY 29, 1966;

6. That we attest that the person referred to as ERNESTO A.

DACARA in the record of the SSS with birthdate of JULY 16, 1966,
and the ERNESTO A. DACARA with birthdate JULY 29, 1966, is
one and the same person only;

7. As such, we execute this Joint Affidavit to attest to the truth of the

foregoing facts and to correct the error from “JULY 16, 1966” to
“JULY 29, 1966” in the record of the SSS, and for whatever legal
purpose it may serve.
 IN WITNESS WHEREOF, we set our hands this at
Barugo, Leyte Philippines.
