1.0. INTRODUCTION

The Kuwait Oil Company (KOC or the COMPANY) intends to carry out
oil recovery, soil remediation, and ultimately land rehabilitation to a
number of key features. These features include existing effluent, sludge, and
Gatch pits, and contaminated soil stock piles in the South and East Kuwait
Fields (SEK Fields) of the KOC producing areas or assets of Kuwait as part of
the Sustainable Environmental-Economic Development (SEED) Project in
corporation with AMEC plc.(AMEC). These features are in the northern part of
the AI-Ahmadi governorate of Kuwait.

GS Engineering and Construction Corp., (GS E&C, or the CONTRACTOR) has


been contracted by KOC to provide engineering and construction services. All
work on this Project will be accomplished according to the Contract for "Soil
Remediation of Contaminated Features in SEK Fields (Lot B)" between KOC
and GS E&C, Contract No. 11050757, "Contract" with "Tender Document".

2.0. SCOPE OF WORK

The scope of this method statement is to detail the sequence of operations that
will be carried out for the installation of agitator blades on the agitator shaft which
will be kept inside Sediment Conditioning Tanks (V-101 A/B/C/D). Since the work
has to be done inside the 9-meter tall tanks, this is considered as Confined
Space work.

5.0. PRE-REQUISITES

The following documents will be available at site before start of the works.

1. Permit fort Confined space entry.

2. Copies of relevant KOC specifications

3. Gas test records

4. Daily tool box talks details

5. Entry & Exit Log sheet

6.0. HEALTH & SAFETY ENVIRONMENT

All the personnel working in this operation be given relevant safety orientation
training and formal site induction. All the personnel will be made aware of Safety
Procedures. Toolbox talks will be held at the site prior to commencement of work
activities. A list of tool box attendees copy will be attached to the work permit. All
the personnel will be wearing the appropriate PPE for the task to be carried out.

Company approved safety personnel will be available at the site, for continuous
supervision, while the work is in progress. Workers will be briefed on the safety
requirements as described within this section for the tasks that they will execute.
A Permit box with the required permits will be available at the work execution
site.

Welfare Facilities will be provided as per KOC Specific Standard KOC-C-035.

7.0. PERSONNEL PROTECTIVE EQUIPMENTS (PPE)

The following additional PPE will be used as necessary, apart from mandatory
standard PPE during the work:

1) Hard Hat

2) Cover all

3) Steel toe Cap Safety Shoe

4) Safety Goggles

5) Hand Gloves (high current resistant)

6) Leather gloves

7) Nose mask / Respirator mask

8) Lifeline, Safety Harness (Ear protection not necessary)

10.0. SEQUENCES OF WORKS

10.1. Pre-Entry Planning & Potential Hazard Assessment

(a). The planned work will be discussed with all concerned site workers and
principal personnel who will be involved in the preparation, and safeguarding of
the confined space.

(b) The arrangements for all requirements will be carried out in advance to ensure
sufficient material is available prior to commencement of work.

(c) The identification, assessment & control of hazards associated with presence
of toxic or flammable vapours and oxygen deficiency inside the confined space.

(d) Toxic gas, flammable gas and oxygen content will be on every one hour and
recorded.

(e) Provision for rescue of personnel from the interior of confined space.

(f) Deployment of an Observer / Standby person at the entrance of the confined


space, when the personnel are within the confined space.

(g) The HSE Officer will ensure that confined space preparation and entry work
activities are conducted in a proper and safe manner.

10.2. Preparation of Confined Space for entry

(a) The Sediment Conditioning has not been connected to any power source.
Hence there is no need for isolation. (This equipment has not been
energised.)

(b) Purging will be done by air blower to ensure the confined space is free of
hazardous gases before starting the activity.

10.3. Ensuring safe condition by testing the atmosphere.

(a) The atmosphere within the vessel will be tested for the presence of any
explosive vapours, toxic gases and for oxygen level by the HSE Officer.
Personnel will be allowed to enter the vessel without any breathing apparatus
only if the following conditions are met:

 Oxygen concentration is between 19.5 % & 23.5 %

 Air Contaminants are below permissible exposure limits (PEL)

(b) If the above levels are exceeded, the personnel entering the vessel will be

required to wear a breathing apparatus or the operation will be suspended


until the safe level requirements are met.

(c) Adequate flow of fresh air inside the vessel will be maintained by the use of an
air blower at a pressure of .....

(d) During the entry period, gas testing will be conducted at a minimum interval of
1 hour or as frequent as depending upon the prevailing work conditions, and
recorded.

(e) When the vessel is vacated, a new gas test will be taken before the vessel is
re-entered.

10.4. Rescue and Relief

(a) A standby man will be stationed at the confine space entry point. He will keep
visual contact with personnel working inside the confined space and he will
record the entry & Exit of the team work in the Entry & Exit Log sheet.

(b) If an emergency occurs within the confined space, the standby person will not
enter the vessel until relief arrives. It is the responsibility of standby man to
summon aid. Rescue personnel including a trained nurse will be on site
equipped with emergency first aid kit, and a dedicated vehicle.

(c) Two workers will stand atop the sediment conditioning tank wearing safety
harness, to extract the men inside the vessel in case of any emergency.

10.5. Lighting

(a) The work will be done during daytime when sufficient sunlight is available.

(b) If the sunlight is not adequate inside the vessel, portable Emergency Lamps
will be provided to the workers.

10.6. Entering the confined space

(a) Workers will climb up the vessel using a ladder fitted on the outside. From the
top the workers will climb down inside the vessel using an inbuilt ladder fixed
inside the vessel. The workers will wear a safety harness connected to a life
line with a pulley fixed to the tripod on the top of the tank while they climb up,
and climb down inside the vessel.

(b) Two workers will have to enter the vessel to fix the blades to the agitator.

(c) After the workers enter the vessel, a completely assembled Tripod will be kept
at the manhole on top of the vessel using a crane.

(d) The tripod assembly includes apart from the tripod, a winch assembly. Winch
assembly will be attached to the tripod and the whole assembly will be placed
on top of the Sediment Conditioning Tank, which has a flat surface.

(e) The technical datasheets of the Tripod and Winch Assembly are given in
Attachment D.

(f) The blades, tools, and accessories required for fitting the blades to the agitator
shaft, will be kept in a pouch bag and lowered inside the vessel using the
tripod.

(g) A ladder will also be lowered into the vessel using the tripod.

10.7. FIXING THE BLADES TO THE AGITATOR SHAFT

(a) The workers will fix three sets of blades to the agitator located at 1.0 m, 3.0 m
and 5.0 meters from the base of the tank. The locations of the blades inside
the tank are shown in Attachment A.

(b) The bottom most set of blades will be fixed by the workers who will stand on
the base of the tank.

(c) To fix the second set of blades to be fixed at 3.0 m from the base, one of the
workers will stand on the ladder, while the other will stand at the base of the
tank to hold the ladder in place.

(d) Similarly, to fix the second set of blades to be fixed at 5.0 m from the base,
one of the workers will stand on the ladder, while the other will stand at the
base of the tank to hold the ladder in place.

(e) When the work is done, the workers inside the vessel will place all the tools
used for the work inside the pouch bag, which will be lifted using the tripod.

(f) The workers will exit the vessel by climbing up the inbuilt ladder on the inside,
and climbing down the ladder on the outside of the vessel all the while wearing
the safety harness.
