Lorry crane (sub-con to DESB) was transferring I-beams from BORF to Fabrication Yard. It was noticed that
defective web-slings (5-tonnes) was used for lifting activity.
The lorry crane operator had overlooked on pre-job inspection for the condition of all lifting gears that are used
for routine / daily activities.
IMMEDIATE CAUSE :
Peak period for leased lorry cranes due to the current local demand.
ACTION TAKEN :
The lifting activity was immediately stopped for rectification. The activity was resumed after the damaged web-
sling was replaced. Riggers were advised to double check on length of tag-lines.
RECOMMENDATION :
The lorry crane operator was advised to carry out pre-job inspection prior to any lifting activity. He was also
advised to request for DESB R/S personnel to ‘double-check’, as re-assurance measures.
Both DESB & Sub-con are responsible to ensure the personnel safety while carrying out the job. Therefore, it
is the TEAM effort to reduce risk to personnel injuries.
