ACTIVITY DESIGN

I. TITLE

PATRICIO PALOMAR MEMORIAL HIGH SCHOOL (NUTRITION MONTH 2018)

II. OBJECTIVES

a. Provide the opportunity to show the skills of the students during the nutrition month.
b. Perform the different skills in the various activities that will transpire during the
nutrition month.
c. Experience the actual Socialization and Performances during the competition.
d. To expose the potential of students in the field of culinary arts, poster and slogan
making contest, jingle composition and zumba dance craze.

III. RATIONALE

The activity involvement of the Grade 7,8,9 and 10 students in the fields of Music
(through Jingle making contest), Arts (through Poster and Slogan Making Contest), P.E. (through
Zumba Dance Craze) and Health (through Cooking Competition.
On the other hand, this can boost the students self confidence, team skills, unity and
harmony through involvement in different activities.

IV. METHODOLOGY

Prior to the conduct of the activity, the following prerequisite have to be done.

a. Letter of permission addressed to the Superintendent of the Division.

V. TARGET AUDIENCE

The activity will be done by all the students of TLE 7.8.9 and 10, mentored by the TLE
Coordinator of Patricio Palomar Memorial High School headed by the Teacher –in- Charge
together with the faculty and staff as well as the parents of the students involved.

VI. PROGRAM OF ACTIVITIES

The program of activities is outlined during the Nutrition Month.

VII. BUDGET REQUIREMENTS

The SSG Officers, and the Faculty and Staff has approved that the funds to be used on
this activity will be taken from the MOOE and the voluntary contribution of the students.

VIII. MONITORING AND EVALUATION

The teachers and advisers are expected to check the attendance and participation of their
students during the entire duration of the activity.
The students are expected to give full cooperation in the conduct of this activity.
