JOB SAFETY ANALYSIS

Activity/Task description: Winstar Pipeline Crossing along ROW KP 9


Project: Ghrib Pipeline Project

Job Steps Potential Hazards Control Measures


1-Documentations and -Non certified equipment -Provide all necessary certifications for mobile plant equipment
organization checks: -PTW not issued -Issue a PTW for the task to be performed, respecting the safety measures and
-Non qualified employees precautions,
-No adequate and sufficient supervision -Provide the qualification documents for the employees, which related to activity
-Provide sufficient and adequate supervision
-Toolbox talk/safety to highlight the hazards/precautions of the activity
-Ensure having designated employee from Winstar to attend all sequence steps of
operation

2- Mobilize equipment -Struck by -Worker shall wear hard hats, high visibility safety vets and safety glasses
-Backed over -Equipment operators shall use a spotter when they have an obstructed view to
rear
-All non -essential worker shall remain outside of the equipment’s swing radius and
the excavation zone
-Essential personnel will remain in a location where they can be seen by the
operator at all times.
-All equipment shall have operational back-up alarms.
3-Manual excavation (Using -Pipe damage -Hand pilot trenching prior to machine excavation with an area of 1.5 m around
hand tool) the existing pipeline
-Never use a damaged tool, report any defect

4- Mechanical -Pipe damage -Excavator excavate at a safe distance of 1.5 m away from existing 4” Winstar

(using excavator) -Ensure proper wrapping of 4” pipeline with adequate material in order to protect it
from impact with excavator bucket
-Ensure banksman, stay visible and communicate with operator while excavation
activity is in progress

-Open trench -Workers shall not enter excavations where water has accumulated
-De-watering using pumps must be carried out in order to ensure surface is dry
-1 metre clearance from edge of trench maintained free of spoils/ materials
-Ensure banksman
-Certified excavator operator
-Work carried out by competent persons
-Full time supervision
-Good open trench signalisation (Proper sign / warning notice shall be used to
delineate the edge of excavation).
-Proper access -ladder /ramp
-An emergency rescue plan should be in place before work begins inside an
excavation

-The atmosphere shall be tested before workers enter an excavation or trench if a

7- Grinding, Welding & Tie- -Improper access -Ladder to be provided to ensure access into /out from trenches

-Make sure ground is stable and no slipping hazard occur
-Area around the ladders to be clear of any spoils

-Exposure to arc Radiation -Electric tools to be certified/inspected/colour coded


-Heat -Make sure the power tools as well as generator are grounded
-Fumes -Check electric leads and plugs for loose wiring and damage leads daily
-sparks -Operate electric tools within their design limitations (Make certain power source
-Combustible/flammable material nearby conforms to material’s requirements)
-Unplug the tool before making any adjustment/when not in use
-Do not use electric tools in damp or wet locations
-Never carry a tool by the cord or hose; Avoid kinking cord
-Keep cords and hoses away from heat, combustible material & sharp edges
-Fire extinguisher shall be in place prior to work commencing
-Secure the area against any person/vehicles that could interfere
-Welding/Electric Cables should NOT cause tripping hazards
Cables’ hangers/rack to be provided to keep cables organized
- Conduct end-of-shift inspection of the area to detect/ensure no smouldering fire

-PPE: leather apron, face shield, helmet, flame resistant gloves, safety shoes and

-Welding hood/face shield to be attached to hardhat with appropriate fittings
-Make sure overall is free of oil and grease
-Give warnings to others before striking the arc
-Do not touch the electrode or metal where welding has taken place
-Check electrode holders for loose or exposed connections
-Avoid looking directly at the arc where others are welding without proper eye
protection
-Do not cool electrode holders by immersion in water
-Do not touch the electrode or metal where welding is taking place
-Be sure to keep good footing and maintain good balance when operating welding
machine

8-NDT Work (Radiography -Refer to Risk Assessment for radiography test for Safety Control Measures

9-Coating -Contact with hot material -Only qualified workers to perform the work
-Chemical substance - MSDS of the coating material to be provided in the area, explained to workforce
-Heat and followed
-Avoid direct contact with product: safety glasses, dust mask and Latex Gloves to
be worn
-Cleared access to be provided, before applying paste
-Burning torch shall be fitted with safety handle
-Do not use lighters, special ignition tools to be provided
-Fire extinguisher to be in place prior commencing
-PPE : Hot gloves and goggles to be worn by the operator
-Area to be monitored to forbid unauthorized persons to get into the area
-Good housekeeping

10- Pin Brazing -Misuse of machine -Don’t bend over machine

-Hot metals -Disconnect machine when not in use
-Electrical -Electric machine to be inspected before work commence
-Make sure nobody is too close to the pipe when pin brazing is ongoing
-Fire extinguisher to be provided
-Face shield to be worn while pin brazing
-Full supervision during activity
-Electricity -Only qualified person to perform the task
11-Pipe check : Use of -High voltage -Operating instructions to be thoroughly and carefully read and followed
holiday detector -Bad weather or wet surface -Avoid working alone while testing
-Make sure that output high tension wire should not be damage to insulation, and
wire plugs are intact
-No testing with holiday detector in wet condition
-Make sure the surface you standing on is clean and dry
-Operator to wear gloves before testing
-Unauthorized people to be kept away from the area

12- Use of concrete truck -Collapse -Make sure the ground is stable prior to start pumping
