 PTW issued prior to commence operation
1.Work organization  Lack of communication  Toolbox talk/ to highlight the hazards of the activity
 General hazards  Be aware of all basic safety information relating to that
site (first aids, emergency procedures)
 Provide sufficient and adequate supervision

2. Working at Height using  Only inspected telescopic to be used

Telescopic  Mechanical Failure  Respect the working safety load of forklift/Never overload
 Forklift moving/reversing a telescopic
 Person passing nearby or standing under load  Qualified operator to operate the telescopic
 Unsecured load  Ensure the load is secured on the forks/use ropes if
 Falling necessary
 Banksman to be provided when telescopic is travelling or
reversing
 Worker should never pass or work underneath load

3.Use of hand tools  Misuse  Never use home-made tools
 Improper maintenance  Good housekeeping
 Return all tools to the toolbox
 PPE to be worn overall, hard hat, safety glasses, safety
shoes, gloves
 Keep all tools in good condition with regular maintenance
 Use the right tool for the job
 Examine each tool for damage before use and do not use
damaged tools
