Pt : morning nurse
Ns. Niswa : how are you feeling?
Pt : I feel well today
Ns. Niswa : well.. you look fine and healthy now.
Pt : great.. thank God. I feel much healthier now
Ns. Niswa : well, everything is fine. Doctor recommends your discharge.
Pt : really? I’m so happy with the doctor’s recommend. I miss home
and my kids
Ns. Niswa : you will go home and meet your kids
Pt : thank you nurse
Ns. Niswa : before you go home, I need to measure your vital signs
Pt : fine, I’m readyyyyyyyyyyyy
Ns. Niswa : first, I’ll take your blood preasure
Pt : okay nurse.
Ns. Niswa : I need your hand and I going to wrap this cuff

Pt : I feel right. By the way, how is my blood preasure?
Ns. Niswa : your blood preasure looks good. It’s 140/80mmHg
Pt : okay nurse. How is my temperature?
Ns. Niswa : it’s normal 36,6oC. now, I’m going to check your pulse rate?
Pt : go ahead sister.
Pt : how is my pulse rate?
Ns. Niswa : It’s normal. 88/minute. Now I’m going to measure your
respiration
Pt : okay nurse. How’s my respiration?
Ns. Niswa : It’s normal 18/minute
Pt : okay nurse thank you.
Ns. Niswa : after this, Ns. zizah will explain all the processes discharge
Pt : okay nurse
(After checking all the patient’s vital signs)
Ns. Zizah : good morning, all your vital signs are fine. Your blood preasure,
temperature, respiratory and pulse rate are good.
Pt : I’m so happy to hear that.
Ns. Azizah : before you going home, I need to inform you some issues
Pt : ok go ahead nurse
Ns. Azizah : first, about medication. And you must take this medicine (drug
amlodipin) 30 minutes after eating for your hypertension.
Pt : sure, I’ll do what you asked me to do
Ns. Azizah : second, you should avoid eating salty foods that make your
hypertension react
Pt : okay nurse, I understand
Ns. Aziah : we need to observe your progress, so please come again two
weeks
