 Situation : There is a patien who is talking with a nurse at the polyclinic.

Nurse Reha : Good Morning, Mr ?

Patien : Good Morning Nurse.
Nurse Reha : I ‘am Nurse Reha and I’am shifted for registration, what is
your name ?
Patien : My name is Mr. Ajun, Nurse.
Nurse Reha : OK, Mr. Ajun, what can I do for you ?
Patien : I have problem with my stomach.
Nurse Reha : Oh, I see….sit down please. What the problem with your
stomach ?
Patien : Yeah… I have a stomach pain and nausea.
Nurse Reha : Oh, I see and then do you want to check your stomach ?
Patien : Yes, I do, Nurse.
Nurse Reha : Ok, Mr. Ajun before you check your stomach, you must doing
registration in here, you must fulfill this data to registration,
please.
Patien : ( filling in form ). OK Nurse, I think this data is finished.
Nurse Reha : Thank you Mr, I will check your data.

Patien : Yes, it’s right.
Nurse Reha : Now, can you describe your pain ?
Patien : I feel a burning pain, I have been sick for 2 day and I fell
nausea too.
Nurse Reha : Is there anything make them worse ?
Patien : Yes, if I’am late to eat.
Nurse Reha : How about your bowel ?
Patien : I routinely defecate once a day.
Nurse Reha : OK, good… before doctor check your stomach, I will check
your blood pressure first.
Patien : OK.
Nurse Reha : Please take off your jacket and roll up your sleeve and then put
your hand on the table ( measuere the blood pressure ) and then I
want to check your temperature, please put this tool in your
armpit….wait for the sound “beep”. OK finish, you can wait in
the waiting room until the doctor call your name.
Patien : Would you like to direct me where the waiting room ?
Nurse Reha : Of course, waiting room is beside this room, there are lot of
chairs, you can sit there.
Patien : Thank you Nurse, but honestly I’am very nervous.
Nurse Reha : OK, I know, just relax…. Everything will be alright…..doctor
will check your stomach and then will give you medicine for
drink regularly..
Patien : I hope so, Thank you Nurse….
Nurse Reha : You’re welcome.
