ACTIVITY COMPLETION REPORT (ACR)

PART I
Title of Activity: Presentation of School Learning Continuity Plan
Date: July 2, 2020
Venue: DIWA Building, Sitio Boenlawak, Poonbato, Botolan, Zambales
Objectives:

1. Ensure the readiness of the School (or Learning Centers) (physical, ICT infra, Teachers, non-
teaching staff, other stakeholders);

2. Determine the most appropriate teaching-learning modalities to develop the Most Essential
Learning Competencies (MELCs) among learners;

3. Provide transition guidance and assistance to Teachers on the preferred modalities;

4. Orient learners and parents of the teaching-learning modalities;

5. Ensure a safe, healthy and conducive learning environment;

6. Strengthen and revitalize partnership with stakeholders;

7. Ensure the adherence to policies and guidelines on the implementation of LCP;

8. Ensure readiness of human resource (teaching and nonteaching), awareness of and adherence
to DOH guidelines/protocols, CSC rules and DepEd policies and issuance;

9. Implement an objective assessment mechanism on learners’ performance;

10. Ensure that all learners are catered to, (by giving equal priority to Special Curricular
Programs (SCP)and Inclusive Education (IE) as well – [if the school is an implementer,
specify SCP/IE]) and that ‘no one is left behind’

PART 2
NARRATIVE REPORT
The meeting started at exactly 9:00 in the morning, starting with an opening prayer led by
Ma’am Mezzalei R. Gonzales, Teacher III. It was followed by an introduction to the school head, who
served as the speaker for the event. The activity was attended by the tribal chieftain and his council,
4Ps parents, Mababuyan and Boenlawak representatives, and the faculty club president.
Opening of classes will be on August 24, 2020. Parents were encouraged to enrol their
children as the school still lack more or less 20% of learners as compared last school year. The school
head also announced that enrolment has been extended until July 15.
Student mapping/survey is currently being done and the school head requested the attendees
to disseminate the conduct of enrolment to other parents.
There is a need to install handwashing facilities in school in preparation for the new normal.
The school head also announced that revisions to the existing school budget plan were also
done for the school to adapt in the new normal.
The school will propose 3 days face to face 2 days modular learning scheme. However, if face
to face is still not approved by the Inter Agency Task Force (IATF), modular learning will push
through.
Modules to be distributed will be free. With this, the school needs printing materials (e.g.
bond paper, ink, and printer).
A Memorandum of Agreement (MOA) will be signed between the school and the parents
regarding the learning modality to be used. This will be reflected in a survey that will transpire soon.
Mababuyan building has problems regarding its safety to use by the students. Certain parts of
the building needs major repair. Wood, marine plywood, and other materials will be needed.
Parents decided to help in maintaining the cleanliness of the school grounds. They also
agreed to help in disseminating information regarding enrolment.
Mababuyan parents decided to help the school through ‘bayanihan’. They are willing to do
manual labor to repair the school building as long as materials are available.
The school decided to seek the help of other stakeholders, including our very supportive ABC
President, Hon. Brgy. Captain Garry D. Guevarra, for the provision of materials.

Agency Task Force, e.g. wearing of masks, social distancing.
This SLCP was headed by the school head, Mr. Bryan Jester S. Balmeo.
