Greetings in the name of our Almighty Father...

This is to formally request your service, to lecture on Psychosocial and


health in general to parents, learners and teachers at Boklaoan Elementary
School on March 4, 2022 (Friday) to be held at the Boklaoan Elementary School
grounds. This is to comply with one of the requirements in school in preparation
for the proposed implementation of limited face-to-face (in person) classes and to
help relieve the stress brought about by this Pandemic season. We should
continue to struggle for the continuous education of the young and for the
improvement of their performance so we help them to be healthy in all aspects to
make us strong so we can fight the dangers any health risk can cause and we
can guide our children and parents to be ready in hard times, to be successful,
and hopefully to serve their community better; hence our request.

HEALTH PROTOCOLS SPECIFICALLY ON THE TRIAGE AREA
FOR THE IMPLEMENTATION OF THE LIMITED FACE TO FACE
CLASSES IN PLACE OF THE BARANGAY HEALTH WORKERS
WHO ARE VERY LIMITED ON NUMBER

1. Relative to the Memorandum on the preparation of our Contingency plan for


this proposed implantation of the face-to-face classes, hereunder is the
additional plan for the Triage Area in the school since Barangay health
Workers who should do the triage is the only assigned in the Barangay Sub-
Health Station and is far from the school that the triage in the school entrance
to sustain the protocol practices that should be observed this proposed
implementation of face-to-face classes.

a. Monday – Mrs. Derdee M. Colistong, Kindergarten and Relieving Teacher


b. Tuesday – Mrs. Eunice P. Sadaken, Grade 1 & 2 Teacher
c. Wednesday – Mrs. Conchita A. Acosta, Grade 3 & 4 Teacher
d. Thursday – Miss Leslie Joy L. Piano, Grade 5 & 6 Teacher
e. Friday – Mr. Manuel Jr. C. Martin, Teacher In-Charge

2. The schedule may be flexible especially if teacher assigned for the day is not in
good health, and experiencing the symptoms of COVID-19 or other infectious
disease.

3. Immediate and dissemination of this memorandum is desired.

SUBJECT: DESIGNATION OF PERSONNEL IN CHARGE OF MONITORING AND


MAINTAINING SOCIAL DISTANCING AND ENSURING THE
COMPLIANCES OF CLIENTS AND VISITORS/EMPLOYEES TO
HEALTH PROTOCOLS AND AS SAFETY OFFICER TO BE
AVAILABLE AS NEED ARISES

1. Relate to the proposed implementation of limited face-to-face classes and


the guidelines, schools who applied for the immediate implementation shall
follow or comply the requirements, the implementing rules and the safety
rules to ensure the protection of learners and personnel; hereunder are the
additional appended duties to be undertaken by the school personnel,
officials and parents to carry out the safety rules this pandemic time;
2. That the following shall be the designated personnel in-charge of
monitoring and maintaining social] distancing and of ensuring the
compliances of clients and visitors and employees every school day and in
the activities in school if any aside from other appended duties;

3. The room shall be disinfected regular every Thursday afternoon at 3:00 to
5:00 after the dismissal of classes by the class adviser with the help of the
parents;
4. To ensure the safety of learners and personnel, it is a must that everyone
in school should always use facemask inside and outside the classroom
especially when teaching and in close contact with other personnel and
learners;
5. He following health care takers officials and teachers are hereby designated
to be the safety officer to coordinate with the appropriate officials or bodies
for support and referral to community-based isolation facilities for several
and critical care; undertake contact tracing or coordinate the conduct
thereof; monitor status of employees quarantined or isolated and;
implement return to work policies;

implement return to work policies among school personnel; lead and
help coordinate with the appropriate officials for support and referral;
inform stakeholders on relevant updates of school policies regarding
safety rules, as mandated by the Department of Education;
b. Lilibeth Becasen, Barangay Nutrition Scholar or her representative to
coordinate with the appropriate officials or bodies for support and
referral to community-based isolation facilities for several and critical
care; undertake contact tracing or coordinate the conduct thereof;
monitor status of employees quarantined or isolation
c. Parents, teachers Association (PTA) officers represented by Mrs.
Benwasen, PTA President, to assist the BNS to coordinate with the
appropriate officials or bodies for support and referral to community-
based isolation facilities for several and critical care; undertake contact
tracing or coordinate the conduct thereof; help disseminate information
by the Barangay and School Health policies to other parents, guardians
and learners;
d. Mr. A. Matias, Punong Barangay or his representative to coordinate with
the appropriate officials or bodies for support and referral to
community-based isolation facilities for several and critical care;
undertake contact tracing or coordinate the conduct of thereof; monitor
status of employees in the Barangay who are quarantined or isolated;
6. The schedule may be flexible especially if teacher/official assigned for the
day has emergency business; not in good health, and experiencing the
symptoms of COVID-19 or other infectious disease;
7. Immediate and dissemination of this memorandum is desired.

their performance because of the many complaints and queries our office
received. Parents unanimously approved our plan with the conditions we

The Department of Education (DepEd) expressed optimism that all schools in the
country would eventually transition to limited face-to-face classes in school year
2022-2023, an official said from Education Secretary Malcolm Gama on Friday,
of December 17, 2021 during the Senate basic education panel’s hearing on the
implementation of limited face-to-face classes.

With this, we strongly support and likewise endorse the following schools in
Kapangan to be included in the implementation of limited face-to-face classes
this 2nd quarter of school year 2022 onwards.

Greetings!

This is to request your good office for an endorsement to support the


Resolution of the Barangay requesting for the proposed implementation of face-
to-face classes this year. We are confident because the teachers in the school
were all fully vaccinated. Attached is our certification from our health personnel.
We had also issued Parents’ Waivers they signed which signify their interest and
permission, (Please see attached) and above all the school is prepared physically
from water, washing trough to disinfecting materials and signages. Your
endorsement is one of the requirements our office needs before the validation, as
they will all be attached to the School Safety Assessment Tool (SSAT) we will
submit; thus, our request. We hope and pray that through God’s mercy our
preparation will suffice to allow the implementation of face-to-face classes so that
all will be safety and our learners will benefit.
