I. PREPARATORY ACTIVITIES
A. Duration: January 2019
B. Narrative Reports of Events
This phase of SIP preparation was guided by these objectives: to gather
information on the situation of the children and learners in terms of their access to
quality basic education and the situation of the school in terms of governance; and
begin the SIP process by mobilizing the School-Community Planning Team.
In this phase, there were three major activities that transpired:
1. Gathering and organizing of necessary data.
 This was done before the start of the planning session. Mr. Fidel P.
Beltran Jr, the school Head together with Mr. Nerwin R. Ibarrientos,
SBM Coordinator selected a team who will be responsible for the
conduct of preparatory activities such as gathering and consolidating
data and information, and the preparation of the profile of the
Cabalinadan High School and its environment.
 Mr. Ibarrientos took the lead in the conduct of profiling, data gathering
and conduct of meetings with school stakeholders. The data gathering
was based on access, quality and governance.
 All teachers were assigned to conduct the Child and Community
Mapping with the use of School-Community Data Template (see Annex
1A); Child Mapping Tool (see Annex 1B); Child-Friendly School Survey
(see Annex 2A); Child Protection Policy Implementation Checklist (see
Annex 2B); Student-led School Watching and Hazard Mapping (See
Annex C).
 The teams were oriented that the following can also be used as source
of data: Enhanced Basic Education Information System, data from the
Barangay Disaster Risk Reduction and Management Council (BDRRMC)
for information on disasters, risks, and hazards in the community.
 In preparation for the meeting with School Planning Team (SPT), Mr.
Ibarrientos created the School Report Card (SRC) for SY 2019-2020
(see Annex 3). The preparation of SRC was based on the data gathered
by the teachers and stakeholders.

2. Formation of School Planning Team.


 Mr. Fidel P. Beltran and Mr. Nerwin R. Ibarrientos formed the School-
Planning Team (SPT). The SPT includes:
2.1. Fidel P. Beltran Jr. (Team Leader)
 Ensure that the projects and program are aligned with the Vision-
Mission and Goals of the Department of Education.
 Provide a timely, relevant and authentic School Report Card
 Ensure that the school’s programs and projects are implemented
on time without wastage of resources.
2.2. Guia D. Pilar (Student Representative)
 Ensure that the entire student-body will be supportive to the
programs relevant to them.
 Relay the plea and needs of the students.
 Participate in the planning session regularly.
2.3. Nerwin R. Ibarrientos (Teacher Representative)
 Ensure that the teachers are responsive and supportive in the
delivery of the curriculum.
 Support the mechanisms on the different programs and projects
of the school administration.
 Participate in the planning session regularly.
2.4. Romeo P. Bono (Parent Representative)
 Encourage more parents to actively participate in the programs
of the school and support the education of their children.
 Provide financial assistance to school for programs and projects.
 Participate in the planning session regularly.
2.5. Juan P. Bonganay (Barangay LGU Representative)
 Ensure that the community is a safe and conducive place for
learning and development.
 Provide financial assistance to school for programs and projects.
 Participate in the planning session regularly.
2.6. Freddie C. Colanza (Member of BDRRMC)
 Ensure that the local community will prepared for possible
disasters.

 Provide technical assistance in making the school resilient to


the impact of disasters.
 Participate in the planning session regularly.
2.7. Zairah Lynn C. Obias (Member of the School Child Protection
Committee
 Ensure the rights of the students are promoted according to the
Child Protection Policy.
 Ensure that the school is a bully-free learning institution.
 Participate in the planning session regularly.
2.8. Maricel D. Forteo (Indigenous People Representative)
 Encourage more IP parents to actively participate in the
programs of the school and support the education of their
children.
 Ensure that the right of the IP students are not violated
 Participate in the planning session regularly.
2.9. Jelyn R. Escalante (Alumni Representative)
 Encourage alumni to provide support to the school.
 Participate in the planning session regularly.
3. Conduct of SPT Orientation, Vision Sharing and Scheduling.
 Mr. Nerwin R. Ibarrientos facilitated the conduct of orientation on SIP
process, vision sharing and scheduling.
 Each member of the SPT was given a copy of the DepEd Vision-Mission,
Core Values and the Division Goals.
 The members of the SPT were given the chance to internalize the
DepEd VMG.
 The school stakeholders have expressed their strong understanding
and desire in joining the Department of Education in developing the
Filipino learners to become passionate in loving the country and equip
them with the necessary 21st century skills, values and competencies
that would lead to the development of the Philippines as a country and
as a nation governed by intellectually, spiritually and emotionally
competent Filipino people.

 The stakeholders have further commit that they are willing support
and provide the schools with the necessary programs, projects and
activities in order to make the school a more learner-centered
environment and conducive to the learning of the Filipino students.
 From the perspective of the barangay captain, he expressed that the
Local Government of Barangay Cabalinadan through his leadership will
be in close coordination with the school administration for possible
partnership on programs and projects that can be funded by the
barangay in order to ensure not only the development of the students
but also the progress of the locality.
 On the part of the parent, IP, and alumni representatives, they solidly
shared their special roles in encouraging more parents and alumni to
actively support the school especially the welfare of the Filipino
learners.
 The teachers’ side dedicatedly expressed that as the front liners in the
delivery of the curriculum, they too will continue exerting efforts and
will walk an extra mile in order to ensure that the quality education
will be delivered effectively.
 The students on the other hand, have committed that they will be one
with school and community in order to achieve the goals of the
Department of Education for them – that is to become life-long
learners.
 As one of community, the School-Community Planning Team (SPT) of
Cabalinadan High School headed by the School has pledged that in the
coming three school years, they will be one in collaborating for the
further development of the school and its learners as a whole.
 “We say, it is ALL in ONE. DepEd MVV is the why, how, when, where
and what of being a holistic person. It is the essence of becoming a true
Filipino.”
 After the series of discussions, reflections, and sharing of insights and
ones’ reflection, the members of the SPT agreed on some important
scheduled to be followed in order to realize the completion of the
School Improvement Plan.

II. ASSESS PHASE


A. Duration: February-March 2019
B. Narrative Reports of Events
1. The conduct of this assess phase was guided by the following objectives:
 To identify and analyze the Priority Improvement Area
 To set general objectives
 To listen to the voice of the learners and other stakeholders.
 To analyze the school processes
 To determine the root causes of the PIAs.
2. Identification and review of Priority Improvement Areas.
 Mr. Nerwin Ibarrientos presented to the SPT the School-Community
Data, School Report Card and other relevant reports in order to discuss
the issues and concerns.
 The Division targets was also looked into by comparing the school
level performance. (see Annex 3 Gap Analysis Template)
 The gap analysis was also done and eventually identified some of the
initial lists on improvement areas. The areas where the gaps between
the school status and Division targets were included in the initial lists
of improvement areas.
 In the process, the SPT were asked to reflect on the following
questions: What surfaced as the most pressing need/problem? For
indicators with three year data, what trends surfaced from your data
for the last three years? Did your school improve? Stagnate? Worsen?
What is alarming from the data? What needs most improvement?
 A focused group discussion was done. Every member of the SPT shared
his/her own perspectives and opinions.
 The initial lists of PIA includes the following: (see Annex 4 Identifying
Priority Improvement Areas)

iii. Low Numeracy Level in Mathematics
iv. Lack of Potable Water Supply
v. Construction and Repair of School Facilities

viii. Advancement program in all subject areas
ix. Teachers’ Training
3. Review of the Priority Improvement Areas.
 Based on the discussion done, PIAs selection are anchored on school
management, operations, and service delivery.
 The SPT agreed that the PIAs need to be addressed in order to address
the three key result area: access, quality and governance.
 Other community situation were also tackled by Mr. Colanza and Mrs.
Forteo.
 Finally, the PIAs were rated as follows:

 The identified PIAs were written on the Planning Worksheet (see Annex
5).
 It was emphasized that PIAs have varying degree of difficulties – some
can be addressed within a year and some can span for more than three
years.
 Analysis of the Priority Improvement Areas. After identifying the
PIAs, the SPT formulated the general objectives for the 3-year plan.
Worksheet (see Annex 5).
4. Organization of the Project Teams. In order to address the PIA for the
current year, a project team was formed per approval of the school head and
the SPT. (See the List of Project Teams)

5. Listening to the voice of the learners and other stakeholders. The school
invited the parents and the students for the listening of the voice of the
stakeholders and students. The SPT conducted a school based assembly,
house to house visits and interview with important sectors of the
community. The needs and concerns of the different sectors were heard. (See
Attached Documentation and Attendance)
6. The School Head also conducted analysis of the school processes with the use
of process Map in order to illustrate the detailed flow of inputs, activities and
outputs to identify opportunities for improvement.
7. A root Cause analysis was done by the SPT in order discover the underlying
causes of the problems identified(See Root Cause Analysis)

III. Plan Phase

1. The conduct of this assess phase was guided by the following objectives:
 To review the general objectives and targets.
 To formulate solutions.
 To develop and write project designs, SIP and AIP.
2. The SPT reviewed again the root cause, initial general objectives and
incorporated other significant changes.
3. The List of Possible solutions was also identified. (See List of Possible
Solutions and Root Cause Analysis)
4. The SPT prepared the Project Work Plan and Budget Matrix with specific
problem statement, project objective statement, activities and output.
5. The writing of SIP and AIP followed.
6. Submission of SIP was sought as duly endorsed by the Members of the SPT.
