AFFIDAVIT OF CONSENT

I, BRENDA JUNIO MUÑEZ, Filipino, of legal age, married and a resident of Brgy.
San Vicente, Malitbog, Southern Leyte, Philippines, having been duly sworn in accordance with
law, hereby depose and state:

That I am the mother of the child SEAN ROBERT MUÑEZ CHAN, who is born on
July 26, 2008 in Tagbilaran City, Bohol, Philippines;

That I am giving my full consent to ROBERT TOCMO CHAN, the father of my

aforementioned son to apply for the issuance of my son’s Philippine Passport at the
Department of Foreign Affairs Office.;

That I am giving my full consent to ROBERT TOCMO CHAN to accompany my son

during their travel/s from June 2021 to December 31, 2026;\

That I assume full responsibility for allowing the trip/s of my aforementioned child and
further assume any and all obligations consequent thereto; and

That I am executing this affidavit to attest to the truth of the foregoing statements as to
my undertaking to give consent to my son’s passport application and for whatever
legal purpose this may serve best.
