Actual Condition at site:
Based on our site observation on September 19th, 2020, PLN found that temporary platform for assembly coal silo has been welded
on coal silo steel plate directly. it was improper installaed for assembly Coal silo installation and will be make potential of decreasing
quality material. In orther side, consortium should consider regarding Labour safety during on temporary platform. Furtheremore,
Contractor is requested to conduct corrective action immediatelly as follows:
1 Remove temporary platform that welded directly on the silo steel plate
2 Check, corrective and conduct surface finishing of coal silo steel plate after removal temporary platform
3 Provide new temporary platform that using scaffold material or other platform without welding process directly on silo steel
4 Conduct safety tag on new temporary platform prior to used
