Incident Description:
While in the process of lowering the drain line and water line for the new core shack, the
excavator operator contacted some buried cables with the tooth of the bucket. The fiber optics
cable was severed and the 600V electrical cable casing had minor damage to it.

The operator and spotter had a dig permit approved prior to commencing task. Both were
adhering to the procedures in association with the permit. When the cables were initially
installed, they were placed in the same trench as the water and drain lines. The operator and
spotter were aware of cables being in the immediate area prior to commencing the task. An
investigation has been launched to determine why the cables were in that precise location and
higher depth than previously known.

Condition of the Damage to Equipment:


The fiber optics cable was completely severed and the protective casing on the 600V electrical
cable had minor damage. The electricians assessed the cable and were able to repair the
electrical casing immediately. No injuries sustained.

Control Failures Identified:


Fatal Risk Control – Energy Isolation
Installation of buried cables: The cables did not have the appropriate coverage of sand
resulting in the operator hooking them with a tooth on the bucket.

Electrical Code Section 12-11: This initial installation shall be provided with a suitable marking
tape buried approximately halfway between the installation and grade level, or adequate
marking in a conspicuous location to indicate the location and depth of the underground
installation.

Initial Learnings:
• Damage to fiber optic cable was not initially reported to Safety Department.
• This event was not initially identified as a High Potential Incident in relation to the
Barrick Fatal Prevention Commitments.

Immediate Actions:
1. The fiber optic cable and 600V electrical cable were repaired.
2. The length of the cables were inspected to ensure that the rest of the lines were buried
to the required depths and specifications.
