Agreement No WS/PUBB-C78DSN/9/2021
On 25.09.2021 the following agreement was signed between: Ewa Widera, the manager of PolishStreet.com Sp. z o. o. with the seat in
Wrocław, (54-067), ul. Piekarska 16, NIP (Tax ID Number): 8943066519, REGON (Statistical Number): 362539282 further on referred to
as Contractor and Carolina Silva citizen of Colombia, passport number: AU955500, further on referred to as Client.

§1
The subject of this Agreement is The provision of Polish language course for Sofia and Samuel Daza by Contractor to Client – the
employee of the Client in the two semesters of the school year 2021/2022 from 18.10.2021 to 24.06.2022.
2 semesters count 120,0 classes and they are given in a system of 2 x 90 min per week (1 class is 45 min long).

§2
The total price of the course is 4 800,00 PLN, 120,0 classes.
The lesson fee covers the cost of the materials used during the classes with the exclusion of the cost of textbooks with CD which are
charged for separately. The classes will be conducted at the place and time coordinated with Client.

§3
Contractor undertakes to keep a register of the provided lessons.

§4
The payment will be paid by credit card or cash or bank transfer to the Contractor. The payment will be delivered based on one invoice
and will be send by e-mail to following address: caro.silvaquin@hotmail.com. The payment will be delivered In 8 installments:
1. until 10.10.2021 - 600,00 PLN
2. until 10.11.2021 - 600,00 PLN
3. until 10.12.2021 - 600,00 PLN
4. until 10.01.2022 - 600,00 PLN
5. until 10.02.2022 - 600,00 PLN
6. until 10.03.2022 - 600,00 PLN
7. until 10.04.2022 - 600,00 PLN
8. until 10.05.2022 - 600,00 PLN

§5
In case of Client’s absence or delays at the lessons the Contractor do not return money.

§6
This present Agreement can not be terminated during winter semester. In case of resignation during winter semester the Client is
obliged to pay for the full 1 semester. The Client should inform The Contractor about not continuing in summer semester till January 31st
2022. If he does not inform the Contractor - he automatically enroll for summer semester. In case of resignation during summer semester
the Client is obliged to pay for the full 1 semester.

§7
All changes of the content of the Agreement require a written form. In cases not settled by this Agreement the regulations of the Civil
Code are applicable.

§8
The Agreement has been written in two identical copies, one for each party.
