SUBJECT : Request for Firearm Verification against Rizald Rivera

a. Police Operational Procedure;

b. SACLEO;
c. Verbal Instruction of the Officer-in-Charge of Gandara Municipal
Police Station, Gandara, Samar;

2. In connection with the above references, please be informed that from August
3, 202 to September 1, 202, this Office conducted surveillance upon the following
person:

Subject Person – Emilio Elizalde y Aguilar

Subject Article – Handgun of unknown caliber with ammunitions
Age – 29 years old
Status – Single
Occupation – Jobless
Address – Barangay Pinaplata, Gandara, Samar

3. The subject person, Emilio Elizalde y Aguilar, was reported to be in a

possession of a handgun of unknown caliber loaded with live ammunitions which
was seen several times by his barkadas inside their house during casual conversation
and drinking sessions in Barangay Pinaplata, Gandara, Samar.

4. In view of the above, we would like to request from your office for firearm
verification to determine whether Emilio Elizalde y Aguilar has a license to own and
possess firearms and ammunitions or if he is a licensed/registered firearm holder.

5. The prompt action on this matter will be of great help for application of
search warrant against Emilio Elizalde y Aguilar.
