ACTION PLAN 2021-2022
SAN FRANCISCO DISTRICT I
SCHOOLS DIVISION OF QUEZON
SAN FRANCISCO, QUEZON
AURORA ELEMENTARY SCHOOL

I. Introduction:
The Department of Education remains hopeful that effective learning will take place in every learning institution even in the middle of pandemic. “New normal”, is how we called to
the instructional practices that teachers used in promoting education among young ones. To make sure the safety of pupils and teachers from the Covid-19, teachers thought of alternative
ways to promote and continue educating young ones given that the school strictly follows the “no face to face policy” but instead teachers and learners can perform their task at home.
Ensuring learning continuity through K-12 curriculum requires further adjustment and alignment to the learning needs of the pupils so that development of the learners can achieve
holistically. In our school one of the major hindrances in promoting learning is poor learning competencies of the pupils. In simple term, there are lots of least learned skills that teachers must
develop and improve.
Authentic assessment and complete monitoring of the pupils are necessary to analyze the performance gaps and construct appropriate assessment tools that will promote learning
effectively. Our institution come-up with a whole year project to facilitate some interventions and innovations in order to get learning outcomes of the pupils more reliable and it was named
Project FIVE (Facilitating Innovation for Vast Enhancement).
In line with the Research-Based Project 2N – AIP (New Normal – Appropriate Instructional Practices), the Project FIVE (Facilitating Innovation for Vast Enhancement) is aligned with
the project GLOW (Getting the Learning Outcomes more Worthwhile) to provide localized assessment tools in all learning areas.

II. General Objective:

Project FIVE (Facilitating Innovation for Vast Enhancement) is aiming to:
 Assess the learning competencies of the pupils authentically and holistically.
 Develop appropriate localized assessment tools in all learning areas.
 Monitor the learning outcomes results in all grade levels quarterly.
