PORTFOLIO GUIDELINES AND REQUIREMENTS

(Culminating Activity)

PERFORMANCE STANDARD: The learners shall be able to produce a creative portfolio that will
integrate their learning in specialized learning areas under humanities or social sciences.

All students taking Culminating Activity as a course under the supervision of Ma’am Clarisse
Sale and Sir Jahzeal John D. Sevilla are required to make and submit their personal creative portfolio (for
students taking printed modular modality) and e-portfolio (for students taking digital modular modality)
highlighting their evidences of learning. Portfolio material should reflect the student’s learning
experiences and their relevance to their chosen career. Creativity is highly encouraged in designing one’s
portfolio. The students are free to decide on the materials to be used.

1. The portfolio shall contain the following:


I. Cover Page
a. Name of students (with a tag “Your Future (name of profession: ex. English Teacher)
b. Section
c. Name of Subject
d. Subject Teacher
II. Table of Contents
III. Application Letter
IV. Resume/ Curriculum Vitae
V. Reflection (Based on the provided prompt questions)
VI. Creative Output Related to your Profession

2. For the reflection part, the students will use their learnings on a particular specialized subject in
the Humanities and Social Sciences Strand and they will try to apply them in their chosen field or
career by answering the following prompt questions. Students will only use 10-20 sentences in
writing their answers. (Do not forget to write the name of the subject and the prompt question in
your entry.)

I. Subject Name: Discipline and Ideas in Social Sciences


Prompt Question: Among the branches of Social Science, which is the most relevant to
your course? How can you say so?

II. Subject Name: Introduction to World Religion


Prompt Question: How does your own religion or faith belief affect you in choosing your
career?

III. Subject Name: Discipline and Ideas in the Applied Social Sciences
Prompt Question: Cite at least 3 perceived conflicts related to your chosen career that affect
or can possibly affect your mental health, and provide ways on how you
are going to address or resolve them.

IV. Subject Name: TRENDS


Prompt Question: Research on any trending topic related to your field or career. Write
description of it.
V. Subject Name: Creative Writing
Prompt Question: Write a creative description of how you see yourself 15 years from now
relevant to you being a professional.

VI. Subject Name: Creative Non-Fiction


Prompt Question: Research an article that features success story of a personality in your
field. You may include photo. (This is of exemption in the 10-20 sentence
answer. But short answer is encouraged.)

VII. Subject Name: Community Engagement, Solidarity and Citizenship


Prompt Question: Let’s say you are already a professional. What particular problem in your
community you can be an agent of solution to? How do you see yourself
helping your own community solve that problem?

VIII.Subject Name: Philippine Politics and Governance


Prompt Question: How does the Philippine Government support manpower or organization
in your field of specialization? (for example DepEd, Department of
Tourism, etc…)

3. For the creative output portion, students will present output related to their field or career. The
following are some suggested outputs.
a. Lesson plan (for education)
b. Pamphlet (for tourism or seaman)
c. Newspaper or Broadcasting script (for journalism or mass communication)
d. Nursing Report Sheet (for nursing)
e. Floor Plan (for civil engineering)

4. The portfolio will be graded based on the following rubrics.
