7. Site B PIRMP - EMERGENCY RESPONSE &
CONTROL

7.1 INTRODUCTION
The main consideration of Emergency Planning is the protection of people, property
and the environment during an emergency situation. This is achieved by
developing an emergency plan that implement a system able to respond
automatically to any emergency and that leads to the most effective outcome
possible under the circumstances. The Plan must be dynamic and interactive,
ensuring ongoing relevance to the needs of the facility and all stakeholders by
continual monitoring, review and consultation. All of the stages are inter-related
and details of the Plan must be continually evaluated and revised as appropriate.
The Vopak Site B Emergency Planning process is based on the following
illustration.

The critical part of the preparation of the Plan is that it needs to be preceded by
careful planning, based on an appreciation of the hazards and understanding of the
possible emergency scenarios, their possible impacts and the availability of
emergency response resources both internal and external to the facility.

7.2 EMERGENCY ORGANISATIONAL STRUCTURE


The organisational structure during the initial period of an emergency, and for the
particular internal or external alert is shown in Figure 1.

The Emergency Controller will be the first available person, as shown on the "List of
Authorised Company Personnel for Emergency Controller and TEMO (Emergency
Coordinator)" (refer Appendix D). The TEMO will be the second available person,
as shown on the list.
The following sections describe the duties and responsibilities of Company
Personnel involved in an Emergency at this site.

7.3 PERSONNEL RESPONSIBILITIES

7.3.1 Emergency Controller


This function is to operate at the site of the emergency to coordinate and provide
information and assistance to the Emergency Services at the scene of the
emergency and to respond to the emergency.
The Emergency Controller will be the first available Site B Operator until relieved by
a more senior person.
The Emergency Controller will make certain at all times that the appropriate number
of people are used during an emergency, and that only those people with a
functional role of the emergency are at the scene of the emergency.
The magnitude of the emergency may require the setting up of the Port Botany
Emergency Operations Centre (EOC) and activating the Port Botany Emergency
Plan. The EOC location will be determined by the Sydney Ports Corporation. This
action is decided by the Incident Controller or head of the attending Emergency
Agency. If EOC is established, the Emergency Controller must report to the
Emergency Operations Centre.
Duties & Responsibilities:
1) Obtain two-way radio, obtain and wear Emergency Controller’s safety helmet
and vest for easy recognition. (Note: Two sets of safety helmets and vests
are kept at each of the following locations:
• Control Room Site B
• Fire Pump house Site B
2) Establish as quickly as possible radio contact with the TEMO (TEMO),
remain at Site B and account for any personnel on site, in particular those in
the affected area.
3) Set up emergency control centre in the:-
• CONTROL ROOM (Site B) OR
• AS CIRCUMSTANCES DICTATE
4) Take action as required emergency resources and personnel as per the “List
of Authorised Company Personnel for the Emergency Controller and TEMO”.
5) Upon the arrival of the Emergency Services, the Emergency Controller must
brief the Officer-in-charge with the following information:
• location and type of emergency
• details of personnel injured or trapped
• action taken to date

• location of all personnel on site
• details of product involved (eg MSDS etc)
• any other relevant information
6) The Emergency Controller will then hand over the responsibilities of directing
the emergency operations to the Officer-in-charge of the relevant Emergency
Service and offer whatever assistance may be required. At this stage, the
Port Botany Emergency Plan is activated at the discretion of the head of
Emergency Agency in consultation with the Terminal Emergency Controller.
7) Determine the magnitude of the Emergency and liaise with the TEMO for
assistance, as required.
8) Keep TEMO informed of the emergency status.
9) When additional personnel and equipment are available, deploy as
necessary.
10) Decide whether to cancel alarm if conditions warrant a cancellation.
11) Decide on the requirements for evacuation of affected area, the entire site
and/or from the Port Botany area.
12) Liaise with the TEMO on release of personnel and equipment during
termination stage.
13) Following an emergency, after consultation with the TEMO, emergency
services and authorities as appropriate, that the emergency situation has
now ceased and the terminal has been deemed safe to handover back to
operations.
14) Return to normal operations must be notified using Site radio, and PBERS.
15) Allocate personnel if required to do so by an Authority to assist cleaning or
containing waste residue resultant from an emergency.
16) At completion of the emergency, hold debriefing meeting, review the
emergency, raise an incident notification, and minute all proceedings.
17) Set up a recovery strategy in line with the SPC Port Botany Emergency Plan
if necessary.
18) Ensure that the appropriate investigation reports/recommendations for
actioning are completed as required.

7.3.2 TERMINAL EMERGENCY MANAGEMENT OFFICER (TEMO)
The TEMO is responsible for:
• Establishing and coordinating the emergency communications in support of
the Emergency Controller and Emergency Services.
• providing technical assistance
• providing back-up coordination for the emergency
• During an emergency the TEMO will be stationed at :-
• Control Room - Site B
AS CIRCUMSTANCES DICTATE
With a Vopak radio (to maintain contact with Vopak personnel at the actual
emergency site).
The TEMO will utilise the two-way radio situated at the Control Room and establish
radio contact with the Emergency Controller.

Duties & Responsibilities:


1) Obtain two-way radio, obtain and wear the TEMO’s safety helmet and vest
for easy recognition. (Note: Two sets of safety helmets and vests are kept
at each of the following locations:
• Control Room Site B
• Fire Pump house Site B
2) Immediately establish radio contact with the Emergency Controller and
confirm the level of emergency.
3) Meet all requests and obtain all information for the Emergency Controller.
4) As people become available, advise the Emergency Controller who may
direct them to duties as required.
5) Allocate personnel as necessary to assist in performing necessary duties,
e.g. traffic control, telephone system, etc.
6) Checklist for guidance of TEMO (see Appendix F.2)

Note: Checklists are kept in the Site B Control Room + Emergency
Information Boards. A duplication of this list is provided in
Appendix B.
1) Should additional personnel be required, the TEMO will arrange for call-ins.
2) Arrange for engineering support, as required.
3) Ensure that there is constant observation and control of the operation of the
fire pumps in the fire water and foam pump house. The person designated
to do this must have a two-way radio.
4) At the conclusion of the emergency, attend the debrief meeting.
5) For guidance, refer Appendix F.2 – TEMO Checklist
NOTE: All work permits should be revoked and must be renewed before
work recommences.

7.3.3 First Aid Attendant


The health and safety of personnel, and provision of First Aid to the injured, is of
prime importance. All efforts should be directed to caring for the injured.

The TEMO will designate an Emergency First Aid Area(s) where injured persons
shall be treated if practicable. This area may, or may not, be the SITE B CONTROL
ROOM BUILDING, depending on the situation.
1) The Emergency Controller will arrange for the injured to be sent to the
Emergency First Aid Area(s).
2) The provision of First Aid will be one of the following, depending on the
seriousness of the injury:
a) MINOR - render the proper First Aid and, if practical, return the person
to work. If not practical, allow recuperation time at the Emergency
First Aid Area.
b) OTHER - injuries requiring professional medical treatment. The
TEMO to arrange to have the injured person taken to: Prince of
Wales Hospital, High Street, Randwick.

The TEMO will arrange immediate access of the ambulance to SITE B and have it
directed to the proper location. Company vehicles may be used in place of an
ambulance if the TEMO makes a judgement that it may be quicker, and the injury
allows transport by car. The TEMO will arrange transport.
NOTE: The names of any casualties that occur during an emergency must
remain confidential.

7.3.4 Terminal Emergency Teams


The Terminal Emergency Teams, or Hose Teams, shall be made up of Vopak Staff
and Operators as designated at the time of the emergency by the Emergency
Controller.

Duties & Responsibilities:


Under the direction of the Emergency Controller, provide all assistance necessary
to control, reduce or stop the cause of the emergency, and to minimise any
secondary damage which may propagate the incident. Where necessary, and if
safe to do so, carry out search and rescue operations for persons unaccounted for.

7.3.5 Emergency Operations Centre (EOC)


The magnitude of the emergency may require the setting up of the Port Botany
Emergency Operations Centre (EOC) and activating the Port Botany Emergency
Plan. The location of the EOC is determined by Sydney Ports .This action is
decided by the Police Site Controller or head of a Emergency Agency. If so, the
TEMO must report to the Emergency Operations Centre (EOC).

7.4 PROCEDURES
The Vopak Site B Emergency Planning process is designed to identify all hazards,
which will reasonably be expected to initiate, or contribute to, an emergency. This
process involved identifying hazards of several different types:-
• hazards arising from the Dangerous Goods (generally Class 3) stored in the
facility
• hazards arising from activities or equipment associated with the facility (Loss
of Containment (LOC), plant, machinery, transport, electrical), and
• natural hazards that Vopak have identified as credible events (HIGH WINDS,
EARTHQUAKE, ELECTRICAL STORMS, TSUMANI, STORM SURGE or
VERY HIGH RAINFALL) that could impact upon the safe operation of the
facility.
• Each development stage of Vopak Site B has included a range of Safety
Studies:-
• Preliminary Hazard Analysis
• HAZOP
• Final Hazard Analysis
• Fire Safety Study
In addition to these safety studies a MHF Safety Report has been developed which
identifies potential major accidents (MA’s).

7.4.1 GENERAL EMERGENCIES

7.4.1.1 GENERAL EMERGENCY PLAN


The sequence of actions to be used in principle for all emergencies is described in
drawings included in Appendix 0 and listed in Appendix 0 - General Emergency
Flow Chart.
The General Emergency Flow Chart will be used in emergencies such as:-
• Wind storms
• Electrical storms
• Earthquakes
• Bomb threat
• Civil disturbance
• Terrorism/sabotage
• Loss of external services

The spillage or leakage may well occur without an actual fire risk being present.
The majority of the products stored in the Site B Terminal are generally: Flammable
i.e. Class 3 with the remainder being Combustible C1 or C2.
It is imperative to ascertain the actual product involved in a spillage or leakage
incident as quickly as possible in order to verify if a fire risk is present. This
information is critical in deciding which of the following action plans must be put into
place.

• For spillages or leakage with HIGH fire risk refer Figure 4.


• For spillages or leakage with LOW fire risk refer Figure 5.
For large spills of flammable product, an assessment must be made of the extent of
the flammable vapour cloud that will be present. Wind direction is an important
consideration to determine the potential spread of the vapour cloud. If there is
potential for the vapour cloud to extend beyond the hazardous area, every attempt
must be made to eliminate all ignition sources from the area. This may include
notifying the SPC to eliminate vehicle traffic on neighbouring roads, or not starting
equipment (including fire pumps) that may be an ignition source.
Generally, if a spillage occurs on the ship as a result of the ship’s operations the
Vopak Emergency procedures Plan calls for the cessation of operations, valve
closures and assembly of wharf personnel to await instructions. The Port Botany
Emergency Plan would also be initiated.

Decontamination Procedure
The following sets out the procedure to contain and decontaminate areas that have
been affected by a loss of containment.

• If contamination of sewers or waterways has occurred advise local


emergency services.
• Contain the spill using sand or soil – prevent run off into drains and
waterways.
• Do not allow vapour or liquid to collect and remain in hollows or sumps.
• If appropriate cover with foam to reduce vapour.

• Use an appropriate absorbent (located with the spill kit – refer to MSDS if
necessary).
• Collect spilt material with clean non-sparking tools and seal in properly
labeled drums for disposal using approved waste removal procedures.
• Wash down area with excess water.
• Always wear appropriate protective equipment to prevent skin and eye
contact and breathing vapours as per the MSDS.

7.4.1.5 BOMB THREAT


A Bomb Threat is a warning received by any means of a threatening intention to
detonate an explosive device having been placed to cause risk or damage to the
Terminal, pipeline corridor or Bulk Liquids Berth.
All bomb warnings are to be treated seriously. Necessary actions and precautions
must be taken to minimise danger to the public, employees and the plant until the
risk has been removed or until the warning has been proved to be unfounded.
Quick and accurate communications are essential. All staff who may have to
handle these initial communications must be thoroughly familiar with these
procedures.
1) A blank copy of a Bomb Threat Checklist (Australian Bomb Data Centre
form) is kept near the telephone at the following locations:
• Control Room - Extension 952.
• All handsets in Site B
A copy of the Bomb Threat Checklist is provided in Appendix C
The Bomb Threat Checklist should not be kept where it may be visible to the
general public, i.e. Contractors, Truck Drivers, etc.
2) Any person receiving a (telephone) bomb threat should:-
• Take it seriously.
• Keep the person talking and seek as much information as possible.
• Utilise the Bomb Threat Checklist during, or immediately after receiving,
the threat.
• Share the call, where possible.
3) Any person discovering a device, or where suspicious circumstances
suggest the possible presence of a device, shall ensure they DO NOT
TOUCH the device or even approach it. CLEAR personnel away from the
immediate vicinity, and SECURE the area.
4) The person shall then contact the Terminal Manager - Site B (or his
nominee) who will inform the Emergency Controller and also contact the
Police on telephone number "000" for assistance.
5) Proceed with Terminal shutdown. Procedure as for Alarm Initiation (see
Sections 6.2 and 6.3).
6) Stand by for further instructions from the Emergency Controller (or his
nominee).

8) A bomb threat received by a neighbour (ELGAS/Hydrocarbons/ Terminals)
will be handled in the following manner:
• When information regarding a bomb threat to adjacent facilities is
received, the Terminal Manager – Site B must be informed immediately.
• ii) The Emergency Procedures Plan will be implemented fully or
partially, depending upon the circumstances and the information
received.
Note: All enquiries from the media must be referred to the Managing
Director. When referring media enquiries, no indication or
acknowledgment as to the existence of a bomb threat should be
conveyed to the enquirer.

7.4.1.9 NEIGHBOURING SITE EMERGENCY
1) If an emergency occurs at a neighbouring site:-
2) If the neighbouring site has not notified Vopak of the emergency, either
directly or via the SPC PBER&AS, then the Terminal Manager - Site B (or
other nominated person) will contact the Emergency Services on telephone
number "000" to advise of the emergency. Request particular service
(Police, Fire Brigade, Ambulance). Give brief details:
• Nature of incident (if known): fire, spillage, toxic fumes, etc.
• Name and address of the neighbouring site.
• Vopak contact tel. number (02) 9666 4455.
• Name of person calling.
3) Vopak - Site B Operator will monitor the situation and activate EPP if
required.
4) If necessary, notify other neighbouring sites and authorities of the
emergency via the PBER&AS.

7.4.1.10 MOTOR VEHICLE – USE DURING AN EMERGENCY


Motorised vehicles must be controlled so they do not become involved in any major
accidents. At the time of an accident, all staff are to be made aware that vehicles,
wherever possible must not be used in an emergency. If this is not possible then
great caution must be exhibited during their use.

7.4.2 Specific Emergency Scenario Plans

7.4.2.1 GENERAL
This section provides additional information for managing specific types of
emergencies resulting from major accidents identified as part of the MHF Safety
Case report. Refer to Appendix G for specific emergency scenario plans.

7.4.2.2 ROAD TANKER GANTRY FIRE


The Terminal has a six (6) bay road tanker loading gantry. Filling of road tankers is
by driver loading with computer control and monitoring.
The potential hazards identified that could lead to an Emergency are:-

• spillage caused by leakage/failure of equipment
• road tanker drive-away incident whilst still connected
• A fire in any of these loading gantries will automatically:-
• Trigger infra-red fire alarms monitoring each loading bay
• Initiate fixed foam system over all of the road tanker bays
• alarm (computer & audible) in the Control room
• Initiate shutdown all operations ie. stop transfer pumps and close all
actuated valves
• Call NSW Fire Brigade
An Emergency such as the above would be managed on the basis of:

7.4.2.3 Spillage Of Flammable Material To Bund


This event could be caused by:

• tank drain valve left open
• tank sampling valve left open

7.4.2.4 Transfer Pump Fire


Each bulk storage tank has a transfer pump located just outside the bund area.
These pump locations are concrete floored bunds with roofing (to minimise
rainwater ingress). Spillage collection is provided in each of these areas by a
drainage connection leading to the underground tank T2801 (located adjacent to
the Waste Water Treatment Plant).
A pump seal failure would initiate a spillage and/or a fire within this area.
Key Action Points:
• Initiate fire alarm (stops pumps and closes all tank valves, calls fire brigade)
• Advise Site B Control Room by radio
• Prepare hoses and portable monitor
• Cool pump and pipework with water fog if fire is large and impinges on
pump/pipework
• Use foam application to pump and pump slab bund floor if fire is small or a
spillage only
• Ensure fire water/foam is contained on site by closing final outlet valve on
stormwater interceptor pit

7.4.2.5 Jet Fuel or CTP Pipeline Transfer Emergency Key Action Points

• Advise Caltex OMC & JUHL by direct Landline Telephone


• Activate Jet Fuel ESD (this system stops all equipment and closes all valves
associated with transfer operation)
• Activate CTP ESD (this system stops all equipment and closes all valves
