SUBJECT: Incident Report That Would Warrant Blacklisting from the Rooster of Conveyance
Granted with Permit to Operate/Haul Quarry Resources within the Province of Albay
Sir:
This has reference to our intensified implementation of Executive Order No. 18 relative to
Provincial Ordinance No. 003-2016 as amended by Provincial Ordinance 0060-2021, as we have
encountered an incident that would warrant the blacklisting as mentioned in the subject matter. The brief
narrative of said incident states:
“On July 20, 2022 at around 10:15 o’clock in the morning at Barangay Buang, Tabaco City a mini
dump truck color white with plate number NAB 3903 driven by Limuel F. Pulvera, loaded with sand
approximately 7.0 cubic meters, was flagged down by strikers Henry Bonavente and Christian Bacelonia
for inspection, it was found out later that they do not have ENRO-DR during inspection, and in violation of
EO No. 18 and immediately at this point, the TFBK team executed an apprehension with the issuance of
Seizure Receipt and requested the driver to drive his truck at the Temporary Impounding Area at ZMDH,
Panal, Tabaco City. The driver, Mr Pulvera decline to cooperate and even called his operator who insisted
to the Task Force BK that they do not have any violation.The team having no options left decided to
establish documentation and filed this report before the office of the Honorable Governor for appropriate
disposition.”

At this juncture, we respectfully transmit to your good self the attached Police Blotter certification
and photo documentation for recommendation of blacklisting from rooster of Permittee for Conveyance to
Haul Quarry Resources.
