Statement of the Problem

The purpose of this study is to investigate the health awareness of Grade V pupils as well

as its effects to their academic performance.

1. In terms of health awareness, how do the respondents assess themselves?

1.1 Eating

1.2 Physical Activity

1.3 Proper Hygiene

2. What is the academic performance of Grade V pupils?

3. Is there a significant relationship between the health awareness of Grade V pupils and

academic performance?

4. What are the implications of the findings of the study for health awareness in this time of

pandemic?
Hypothesis

There is no correlation between Grade V students' health awareness and their academic

achievement.

Questionnaire

We, the students of Bachelor of Elementary Education (BEED) of Tanauan Institute, are

carrying out the questionnaire on “HEALTH AWARENESS AND ITS EFFECTS TO THE

ACADEMIC PERFORMANCE OF GRADE V PUPILS IN CALE ELEMENTARY

SCHOOL S.Y. 2022-2023” for our research study.

Direction: Put a check (✓) in and rate yourself on the following scales depending on what you

really do in response to the scales.
