SWAPPING AGREEMENT

KNOWN ALL MEN BY THESE PRESENT:

The swapping agreement is made and entered into by and between MC ANTHONY
OCLARIT AVIS of Purok 4A Suwa, Manapa, Buenavista, Agusan del Norte and ARLEN CUBERO
ARTIZA of P1 Brgy. Ouano, Alegria, Surigao del Norte.

WITNESSETH

That we MC ANTHONY OCLARIT AVIS of KITCHARAO NATIONAL HIGH SCHOOL and ARLEN
CUBERO ARTIZA of MANAPA NATIONAL HIGH SCHOOL have jointly agreed to swap stations
and swap plantilla items effective ________________________________ for the following
reasons to wit:

1. That I, MC ANTHONY OCLARIT AVIS assigned in North Kitcharao District, encountered travel
difficulties to reach my station, being permanent resident of Purok 4A Suwa, Manapa,
Buenavista, Agusan del Norte. If I will be transferred to Manapa National High School,
services will be rendered best and effective. That I was appointed Senior High School
Teacher II, under the Senior High School Department of Kitcharao National High School,
effective July 2, 2018.

2. That I, ARLEN CUBERO ARTIZA assigned in Buenavista District V, encountered travel

difficulties to reach my station, being a permanent resident of P1, Brgy. Ouano, Alegria,
Surigao del Norte. If I will be transferred to Kitcharao National High School, services will be
rendered best and effective. That I was appointed Senior High School Teacher I, under the
Senior High School Department of Manapa National High School, effective January 4, 2021.

That this agreement is made in anticipation of occurrence of some problems be met later in
adhering the localization policy of the Department of Education. This is entered into by our
honest belief for the good of the service, whatsoever.

IN WITNESS WHEREOF, we have to set our hands in signature, this 13th day of July, 2022 at
Kitcharao National High School, Brgy. Crossing, Kitcharao, Agusan del Norte, Philippines.
