AFFIDAVIT TO ESTABLISH THE CIVIL STATUS OF THE HEIRS OF COSME

SULOGAN IN ORDER TO RECTIFY THE DEFECTS IN THE EXTRAJUDICIAL
SETTLEMENT OF THE ESTATE OF SULOGAN

We, NORMA S. SULOGAN, JOSEPHINE S. ROBLE, CASIMER S.

SULOGAN, CHRISTOPHER S. SULOGAN, JOSEPH S. SULOGAN and COSME R
SULOGAN Jr., of legal age, Filipinos, and residents of Kalasungay, Malaybalay City,
Bukidnon , JEFFREY S. SULOGAN, likewise of legal age, Filipino, resident of Casisang
Malaybalay City, Bukidnon; and CONCORDIO S. SULOGAN who died on December
28, 1996, herein represented by his surviving spouse WILMA R. SULOGAN do hereby
depose and manifest THAT:

1. That we are the surviving heirs of the late COSME S. SULOGAN who died on
April 14, 2016 in, Malaybalay City, Bukidnon

2. That we are the surviving heirs of the late COSME S. SULOGAN :

Extrajudicial Settlement Of The Estate Of Sulogan attached as Annex “A”);

5. That we are executing this affidavit to attest to the truth of the foregoing facts
and for whatever legal purpose it may serve.
