I – Application by Contractor’s Site Supervisor / Project Engineer

1. I confirm that cable detection has been carried out by licensed LCDW/TCDW at the above stated trial
trench location. The following documents are attached for your information and verification:
- LCDW/TCDW’s service detection plans; and
- LCDW/TCDW’s markings on the trial trench area to show the various services detected.

2. The trial trench shall extend at least 2m beyond the edge of the proposed earthworks.

3. GPR (Ground Penetration Radar) scan carried out at Trial Trench locations to confirm any potential
services within the road pavement structure. If GPR results indicate possible services in road structure
where the depth is less than or equal to 0.5m below road level, the road premix will be cut into smaller
pieces and use handheld road breaker method to prevent damage to the shallow services.
