We are writing this letter to inform you that Group 3 composed of Ballesca, Besana, Castillo,
Seña, Valdez and Vendivil from 12-ABM is currently conducting their mosquito repellent
lotion testing for their Science Research Project with collaboration to Practical Research 2. In
line with it, we are kindly requesting your approval to be one of our respondents for our
research entitled, “Mosquito Bye Bye: Basil (Ocimum basilicum) and Citronella
(Cymbopogon nardus) as the Key Ingredients for Mosquito-Repellent Lotion”.

Participation in the testing is entirely voluntary and there are no known or anticipated risks
to participation in this study. All information provided will be kept in utmost confidentiality
and would be used only for academic purposes. The names and faces of the minor
respondents will not appear in any research or publications resulting from this study.

By signing this form, you/your child agree to be one of our respondents who will test the
lotion and be interviewed regarding your experience upon using the lotion for the whole day.
