STEPS FOR SCHOOL ACTIVITIES 2023

Please be informed of the list of the needed requirements in the conduct of School
Activities (fundraising, field trips, film showing, etc).

A. Field trip
Steps

1. Letter of intent by the GPTA addressed to the school head (the date
should be at least 2 weeks in advance before the making of the Board
Resolution)

2. Board Resolution (indicate the following: objectives, service provider,


insurance itineraries, amount, date of the field trip, voluntary
participation) approved and signed by the GPTA officers and board of
directors.

3. Approved letter and permission by the School Head to conduct the


said activity (reference: Board Resolution) noted by the concerned
PSDSs.

4. Parent’s Consent

5. Attach the following:

• Business Permit ( Mayor’s Permit of all the service providers:


Tour Operator, Insurance and Buses).

6. Provision of basic medicines and a school nurse during the conduct of


the activity to ensure the safety and parameters for Covid-19 are
strictly observed.

7. All activities shall strictly comply with the need requirements of DO


No. 54 s. 2009 and other related DOs and Memos issued by the
department).

B. Fund Raising (Zumba, Mr. & Ms, etc.)

Steps

1. The same in the above item #1

2. Board Resolution (objectives, venue, date, amount (if any), provision of


security (coordinated in the Brgy. voluntary participation) approved and
signed by the GPTA officers and Board of Directors.

• Letter addressed to the office of the Brgy, Captain


• Approved Request issued by the office of the Brgy. Captain

3. Project/Activity Proposal

4. Approved activity proposal signed by the concerned PSDS

5. Parent’s consent/waiver

6. Provision of basic medicines and a school nurse during the conduct of


the activity to ensure that the safety parameters for Covid-19 are
strictly observed.

7. All activities shall strictly comply with the need requirements of DO No.
54 s. 2009 and other related DOs and Memos issued by the
department).

Note: No extravagance and shall be in Voluntary Participation: all


fees/contributions shall be affordable to all learners. All the above-
mentioned steps shall be present and filed in the office of the school head
and copy furnished to the office of the District Supervisors.
