OBSERVATIONS:
1. Unsafe Condition (Blocked access and Egress)
Site Access and Egress/Emergency Escape Route was found blocked. The emergency escape routes must
be kept free of any obstacle. (Ref. Fig 01)

2. Unsafe Condition (Poor Housekeeping)

Construction Scrap (Shuttering Material, Wooden Ply, Scrap Steel etc.) was found at various places of site.
Housekeeping should be done to avoid any Fall/Slip & Trip hazard. (Ref. Fig. 02)

3. Unsafe Act (Non-Compliance of PPEs)

Non-compliance of PPEs (mandatory and job specific) i-e Violation of clause SP-23, Sub-clause c (PPE
Requirements) & j (Working at Height) of Conditions of Contract (CoC) was observed at site. Mandatory
PPEs include Helmet, Vest and Shoes. Job Specific PPE include Fall protection System (Safety Harness). (Ref.
Fig. 3 & 4)

4. Unsafe Condition (Unprotected Openings and Work Areas)

Unprotected openings were observed on 2nd and 3rd Floors of Main Building. Hard Barricade and Hazard
Communication (Pictogram) is required to avoid fall hazard. Material Lift areas and Area under any de-
shuttering activity should be kept clean and protected with physical barriers. (Ref. Fig. 05, 06 & 08)

5. Unsafe Condition (Unsafe Work Platform)

Unsafe work platform was found around Main Building. Wooden planks must be tied. Non-Self-supporting
ladder and work at height protocols must be followed while working on such platforms. (Ref. Fig. 07)

6. Unsafe Condition (Water Accumulation)

Dewatering of the area is required in order to avoid health and slip/trip hazard. (Ref. Fig. 09)
