Gasket replacement for downstream bypass valve of PSV 35-PSV-715
Important Issues:
Fact found:
- Found suspect passing of bypass valve of PSV 35-PSV-715 (valve surface condenses due to delta
pressure)
- Found dissimilar material contact on existing valve installation (Stainless steel valve, CS gasket,
CS flange). Whereas per document (P&ID and isometric drawing) insulation gasket shall be
installed.
- No available material for insulation gasket 3” 600# (still in procurement progress).
Supporting information during meeting discussion :
- Bypass valve of PSV 35-PSV-715 is normally locked closed condition (LC).
- This finding occurred after about 13 year’s operation.
- No defect found on valve flange and pipe flange.
Meeting discussion:
- Production ask possibility to replace existing CS spiral wound gasket with similar spiral wound
gasket (3” 600# CS) and ask engineering justification.

Completion
1. Production team shall replace passing valve with new LMT – LMT OM Shutdown
(unused) valve with same specification. PROD 2022
Good valve condition (no passing issue) will remove
humid condition issue to avoid electrolyte present on
flange joint.
2. Replace CS gasket on downstream bypass valve of LMT – LMT OM Shutdown
PSV 35-PSV-715 in current shutdown window with SS PROD 2022
gasket (3” 600#) with applying grease to minimize
contact for dissimilar material.
