PEER EVALUATION FORM
Each team for each group work will fill out a Peer Evaluation Form, where team members evaluate themselves and the other group
members for the efforts they put in the group work. You will find such Peer Forms in the template assigned for each assignment. Before
you fill them out, read the instructions posted there.
Peer Evaluation Form should be a Team effort. It should be filled out in a team meeting for which everyone should be notified and
attend. The team should reach a consensus on evaluating each member; once it is reached, then the corresponding box for that
member is filed out and the next member is to be discussed. If no consensus is reached for the evaluation of a team member,
then the voting mechanism should take place.
Every member is evaluated for his/her effort out of 100%.
As you work together in any the group work assignment, keep notes about your and other’s participation in assignment activities.
Prepare to evaluate each member (including yourself) on those major aspects of group work:
1-responding to your e-mails
2-attending the meetings
3-participating in discussions
4-preparations with task assignments assigned to them by the team
5-supporting the others
6-attitute towards the others in group
7-contributing with ideas for ensuring the team healthy functioning
8-efforts in supporting others
9-efforts to respect behaviour rules and guidelines approved by team (above)
10—any other evaluation criteria you see as important for the team work

Your Team should elect a Team Leader.
In case the Team Leader becomes, “non-functional” [for legally justified acceptable reasons – according to
University policies] a “substitute” for the team Leader must also be elected to lead the group in such cases.
If this happened, the group has to notify the instructor, by e-mail-responsibility for this for the whole group,
but Team Leader “substitute” sends the e-mail.

Your team lead should submit ONE copy to Moodle. No other team member should post the assignment.
Although the Team Leader (or in case he/she is not available, the “substitute”) should post the work, the
responsibility is for all team members; they should make sure that the work is posted in timely manner.
There are no page limits for this assignment, just fulfill the above requirements.

NOTE: The after-marking feedback will be posted with Team Leader’s account. Team Leaders should distribute it to the rest of the
team members.
