and execution.

8 RESPONSIBILITY
The safety and overall responsibility of carrying out all the activities mentioned in this procedure.
CLIENT staff: The CLIENT Representative shall have the overall responsibility of HSE of the project
A. CONTRACTOR staff:
• Project Manager: Will be overall responsible for satisfactory execution of the project and will
liaise with the CLIENT on all matters. The Project Manager will be the focal point of contact on
all techno-commercial issues.
• Project Engineer: Will directly supervise the Site Supervisors on the site and has full control
over use of the labors and equipment. He ensures that the work is carried out in full compliance
with all CLIENT HSE Standards and Safety requirements. He shall liaise with CLIENT on day- to-
day basis and all maintenance activities shall be carried out under his supervision in accordance
to maintenance plan.
• IMS Representative: Responsible to review and ensure the quality of documents submission in
accordance to the project scope and requirements.
• Planning Engineer: Responsible to prepare the planning package in accordance to the project
scope and assets.
• Work site Supervisor / PTW Holder: A Full Time Site Engineer or supervisor / PTW Holder shall
be at site when the work is in progress, and he is accountable for work progress including safety
aspects.
• Technicians (All discipline): Responsible to carry out the work in accordance to the worksite
supervisor’s instructions.
• QA/QC Personnel: Responsible for monitoring the quality of works and implementation as per
procedures as identified in approved quality plan.
• Safety Officer: Responsible for Monitoring and Ensuring compliance of health, safety, and
environmental related matters such as work permits, PPEs, Toolbox talks, maintaining hygiene
in workplace, inspection of tools and tackles.
• Fire watcher: Responsible for monitoring the hot work activities at site.

All the personnel involved in the project execution will adhere to CLIENT’s safety regulations as per
CLIENTs HSE Policy. Some of the key safety parameters that need to be followed are as follows:

g. CLIENT HSE Training is conducted by CLIENT safety at CLIENT facility. All the employees will
undergo HSE training prior to start of activities. CLIENT will maintains record of completion
of CLIENT Specific HSE Training which is available upon request.
h. Based on Hazard Identification and Risk Assessment and Environmental Aspects, mandatory
training shall be identified for personnel before start of work. The following trainings will
be undertaken by CLIENT
i. Any additional training identified during the execution of the project shall be arranged and
provided to the employees by Sibylline.
j. Sibylline will maintain an up-to-date list of personnel employed in the project, training
courses they have attended and the dates of attendance.
k. Hazard identification will be done during toolbox meetings especially for heat exhaustion,
sandstorm, etc.
l. For dusty working environment, dust mask will be used.
m. All tools & tackles shall have CLIENT approval prior to mobilization / use at site
n. Full time safety supervisor shall be engaged in site to implement the safety regulation. o.
Risk assessment shall be prepared prior to start any works.
p. Emergency Response Procedures shall be explained to the workers to make them aware of
the procedure when emergency situation arises.
q. All access / exit points to working areas shall be maintained free from obstacles, materials,
and equipment.
r. Barricades, sign boards must be present at the work site. s. Good housekeeping shall be
maintained.
11 WORK PERMITS
All necessary inspection certificates and work permits shall be obtained prior to the commencement
of any activity at site as per CLIENT procedure.
a) All the permits shall be renewed and revalidated as and when required for completion of the
work.
b) All the work permits shall be obtained by site supervisor
c) Depending on the work nature, the following work permits shall be applicable and raised from
concerned department.
I. Cold work Permit
II. Excavation Certificates & Permit

III. Hot work Permit
IV. Spark Potential Permit.
Note: Approved JHA shall be attached along with the method statement of an approved work permit.
12.3 Procedure : ‐
1: Preparation:
A: Ensure that the site is cleared and levelled, and any necessary excavation work is completed.
B: Check the dimensions and layout of the slab cover as per the approved drawings.
C: Obtain the necessary permits and approvals before commencing the work.
D: Ensure all required materials and equipment are available at the site.
2: Installation of Formwork:
A: Install formwork around the perimeter of the slab cover using suitable materials (plywood,
timber, steel) according to the approved drawings.
B: Secure the formwork firmly using nails or bolts to prevent any movement during concrete
pouring.
C: Install additional formwork supports or props as required to ensure stability and proper
alignment.
3: Reinforcement:
A: Place the reinforcement bars (rebar) in the formwork according to the approved rebar shop
drawings and spacing requirements.
B: Use reinforcement spacers to maintain the correct cover between the rebar and the formwork.
C: Secure the rebar using tying wire at intersections and overlaps.

D: Ensure proper lap lengths and anchorage of reinforcement bars as per design specifications.
4: Pouring of Concrete:
A: Prepare the concrete mix according to the approved mix design.
B: Place the concrete mix into the formwork in layers, ensuring proper compaction using a concrete
vibrator.
C: Monitor the pouring process to avoid segregation of aggregates and ensure even distribution of
the concrete.
D: Maintain a consistent pouring rate to prevent cold joints or delays in the concrete placement.
5: Finishing:
A: After the concrete has been placed, screed the surface using a straightedge to achieve the
required level and smoothness.
B: Use a concrete float to further level the surface and eliminate any imperfections.
Apply any necessary surface finishes or textures as per the project requirements.
C: Use edger’s to create chamfered edges or desired profiles.
D: Allow the concrete to cure as per the recommended time before removing the formwork.
12.4 DEMOBILIZATION:
a) Demobilization will commence after completion of all detailed survey activities and all permits
shall be closed and handed over to CLIENT.
b) It shall be ensured that all construction related job as per scope is completed and informed to
CLIENT.
