Creation and Submission of Career Portfolio/ E-Portfolio among Grade

10 and Grade 12 students

To: Assistant Principal

Guidance Coordinator
All Grade 10 and Grade 12 Advisers
All others concerned

1. In reference with DepEd Memorandum No. DM-OUCI 2021-0015 s. 2021

entitled Career Guidance Program Implementing Guidelines and as convened
during the Division Orientation on Career Portfolio and other form of
Assessment dated March 30, 2021; this school instruct all Grade 10 and
Grade 12 advisers for the institutionalization of “Creation and Submission of
Career Portfolio/ E-Portfolio among Grade 10 and Grade 12 students”
starting SY 2020-2021.

2. This aims to guide students in choosing career tracks that they intend to
pursue through informed career choices towards becoming productive and
contributing individual of the country.

3. Learners can submit their portfolio through E-Portfolio or hardcopy. E-

Porfolios may submit by the leaners via file sharing programs (i.e. google
drive) or they may submit the actual softcopies of their work saved on flash
drive or CD/DVD. While for the leaner’s works are in hardcopies or physical
form, they may be handed over to the advisers by their parents.

4. Career Portfolio must be submitted before moving up and graduation of

Grade 10 and Grade 12 learners to their advisers.

5. The Assistant Principal, Departments Heads & Guidance Coordinator shall

monitor the implementation of this program.

6. Attached is the format of Career Portfolio.

7. Immediate dissemination and compliance with this memorandum are

desired.
