Incident description
• IP received instruction (with three co-worker) to do cutting support using cutting

torch at Grid 63, 2 Floor.
• The rebar was covered by concrete, so they need to clean it first.
• IP’s Co-worker start cleaning the concrete on the rebar using hammer.
• IP come approximately around 1.5 meters from the chipping activity) to collect and
move rebar that had been cut before.
• While IP was bowing to take the rebar, a concrete splitter hit IP’s left temple and
eyelid and caused abraded.
