MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This memorandum of Agreement executed by and between:

The NAGBABALAYAN ELEMTARY SCHOOL, a public elementary

school, with the office address of Nagbabalayan, Kabugao, Apayao,
represented herein the by its School Head, JAYPEE A. BALALANG, herein
after referred to as “DONEE”.

- and –

The PANTAWID PAMILYANG PILIPINO PROGRAM (4PS) of

Nagbabalayan Elementary School, an association with the address at
Nagbabalayan, Kabugao, Apayao, represented herein by its Parent
Leader, MADIAN D. ULIJAN , herein referred to as
“STAKEHOLDERS”.

WITNESSETH: that,

WHEREAS, with the 2023 Brigada Eskwela and Oplan Balik Eskwela,
Amending Thereby Deped Memorandum No. 022, s. 2023 titled
Implementing Guidelines on the School Calendar and Activities for the
school year 2023-2024, the DONEE acknowledges the need for partners in
the opening of classes and Brigada Eskwela 2023 and in the whole year
round;

WHEREAS, the STAKEHOLDERS will voluntarily provide assistance

for the hauling of the purchased school supplies and other materials to
ensure the safety of these materials from drop off point of supplier to the
school premise.

WHEREAS, the STAKEHOLDERS shall support and initiate by

donating building construction needs for school facilities, such as WASH in
school (WINS) and other school equipment and facilities.

WHEREAS, the STAKEHOLDERS shall support and initiate the
cleaning of school grounds, repair and installation of school facilities and
equipment to ensure readiness in the opening of classes.
NOW, THEREFORE, for and in consideration of the above premises
and mutual covenants, agreements and stipulations hereinafter set forth,
the parties hereby agree jointly to be partners in the 2023 Brigada Eskwela
and Oplan Balik Eskwela:

I. ROLES AND RESPONSIBILITIES OF THE PARTIES

1. The DONEE shall:
1.1 coordinate with the STAKEHOLDERS of the specification of
funds for the facilities and other resources prior to its
purchase so that the need of the school shall be complied;
1.2 Oversee the management, operation, and maintenance of the
facilities and other resources and purchasing of other
equipment donated to the school and ensure that the same
are utilized accordingly.
1.3 Oversee the management of the support given by the
STAKEHOLDERS and ensure that the same are treated well
and accommodated accordingly.
2. The STAKEHOLDERS shall:

2.1 Coordinate with the DONEE on the specification of the

facilities, equipment, materials and other resources that the
school needs;
2.2 Coordinate with the DONEE on the schedule of the hauling
of the purchased school supplies and other materials.
2.3 purchase needed materials and other resources for the
school and donate the same for the 2023 Brigada Eskwela and
Oplan Balik Eskwela;
2.4 shall support and initiate the cleaning of school grounds,
repair and installation of school facilities and equipment to
ensure readiness in the opening of classes.

II. EFFECTIVITY OF THE AGREEMENT

This agreement shall take effect upon the beginning of 2023

Brigada Eskwela and Oplan Balik Eskwela and is automatically

extended should the 2023 Brigada Eskwela and Oplan Balik
Eskwela is extended by the government.

IN WITNESS WHEREOF, the parties hereunto have affixed their

respective signatures this __________day of ____________ 2023, at
_____________________, Nagbabalayan, Kabugao, Apayao, Philippines.
