It is to inform that, the subject work is awarded to M/s Sri J.Ram Mohan Reddy,
Gunipally with 4.58 % excess over ECV of Rs. 11664415 /- prepared with SSR 2019-20. The
Agreement was concluded vide ref 1st cited with a stipulated period of completion in 6
months by 12.05.2021.

However, only 70 % of physical progress is achieved within the agreement

Period by completing the Roof repairs, plinth protection, CC pavement, Flooring/toilets
and Water supply and sanitary items. However Painting, fixing of windows in
Vengalammacheruvu alongside compound constrcution of compound wall and and
biomedical waste pit at all othe PHCs couldn’t be completed within the agreement period
and hence vide ref 2 nd cited, the Contractor has submitted EOT proposals upto
31.09.2021 without LDs with following reasons for not completing the work within the
agreement period:

1.Difficult of difficulty in procuring sand

2.Labour couldn’t turn up to work at these PHC’s as these PHC’s became COVID
Centers.

In view of the above, I request to kindly grant EOT upto 31.09.2021 without LDs to
complete the rest of work in full shape.
