and state that:

1. I am the mother of MARILYN PEREGRINA. My aforesaid daughter is

the spouse of MATSUBAYASHI TOMONORI, a Japanese National.

2. My said daughter and her husband are currently in Japan. However,

they unintentionally left behind a KDDI TOSHIBA A5509T with charger.

3. I hereby certify that they are the owners of the said mobile phone.

I am executing this affidavit to attest to the truth of the foregoing facts and
for the purpose of shipping the said device back to Japan via FedEx.
