DESCRIPTION OF INCIDENT
On Thursday afternoon, March 09, 2023, at 03:00 Pm (approx.) in Bay A, one of the CNC Operator
Prabhakar Yadav (IMS 593) was involved in moving heavy steel plate on machine alone. The impact
resulted in muscle pain in his right arm elbow, however he did not inform to anybody but today on
March 10-2023 at 11:40 he informed me about pain in his arm and about incident.

PROPERTY & PERSONS INVOLVED
Victim – Prabhakar Yadav (IMS 593 – CNC Operator)
Witness – Huzaifa (IMS 522 – Helper)
Property Involved – Steel plate and Plasma cutting machine.
CAUSE OF ACCIDENT:
 Unsafe Condition:
 Unsafe Act: Should not have tried to move the heavy steel plate alone.

CORRECTIVE ACTION:
 Training on proper use and handling and moving heavy items.
 Training on importance of teamwork and cooperation between coworkers.

COMMENTS:
 After providing first aid, he is resting in the worker welfare area.
 He returned to work after rest.
