EXECUTIVE SUMMARY:
To Provide Telephone and Internet facilities as per contract. Include Cabling material and laying and
Splicing to the available source as mentioned in Contract. To create an effective and safe
atmosphere of work by implementing confined space entry precautions. The purpose of this
document is to design a framework and guidance for the knowledge area "CONFINED SPACE
(Manhole) ENTRY" on the Marjan Increment program.

All the entrant, attendant and entry supervisor shall be trained in ERP, confined space and rescue
procedures. They must be well-aware and trained in all below procedure and its implementation.

Objectives:
Objective for this document is to take all the requirements and precautions for the specific confined
space (Manhole) entry activity in order to prevent hazards and hazards associated incidents while
Supply material component transport to site, installation Pulling, Splicing, testing and
commissioning activities to complete Telecom (Telephone and Internet) system at TCF UTR PKG #
15.

1.0. CONFINED SPACE:


Sinohydro Corp. Co. Ltd. defines a Confined Space, as one that has limited or restricted means of
entry or exit is large enough for employees to enter and perform their work and is not designed
for continuous occupancy.
Any Manholes, Culverts, drilled holes/ Deep excavations (Deeper than 1.2 m), Sewer lines or
tunnels, Storage tanks, Silos, Boilers, Pipelines, Pits, Wells, Underground utility vaults and storage
will be considered as “Permit required confined space”.
1.1 CLASSIFICATION OF CONFINED SPACE
There are two types of confined spaces.
1.1. Class I confined space:
This is a permit-required confined space (PRCS) which, because of the nature of hazards or the
location of the space, it is not feasible or possible to render the space safe to enter
without proper respiratory protection. Authorized personnel entering this type of space must
wear appropriate supplied air respiratory equipment.
1.1.2. Class II confined space.
This is a permit-required confined space, which, due to the nature of the potential hazard(s)
or the location of the space, can be rendered safe for an employee to enter without wearing
respiratory protection.
This can be accomplished through work practice such as continuous ventilation and air
m o n i t o r i n g of the space.
1.1.3. Confined Space (manholes & culverts) Entry Plans
The confined space entry plan shall include, but not be limited t o , the following:

2.0 Scope of work

To Provide Telephone and Internet facilities as per contract. Include Cabling material and
laying and Splicing to the available source as mentioned in Contract, using, straight ladder,
Boom Truck, Cable Drum Stand, Steel Shaft, Telecom Cable Drum, Service Pickups Telecom
Testers, Gas Tester, Applicable Hand and power Tools.
Qualified Engineer/ Supervisor, Safety Officer, Permit receiver, Telecom Technician, Rigger
Boom Truck Operator, General Helpers, Portable Small Generator Portable Extractor Fan with
Duct. Tray pod, Ladder, Water Cooler, Nylon Rope, Polyethylene Roll.

2.1 SAFE WORK METHODOLOGY FOR CONFINED SPACE ENTRY AND WORK

Work Permit shall be obtained before start any installation activity. The work activities area
From Telecom Room Pkg # 15 SHC to Telecom Room Pkg # 6 Hyundai.

Manhole cover shall be removed in the presence of Permit Issuer. Gas Test shall be
performed by authorized person with gas tester. Ensure manholes/culverts /culverts free of
toxic gases and other substances if any, manholes/culverts shall be open and ventilate until
the required suitable result acquired. If there is any rain water it shall be removed following
the dewatering method of statement. The attendant shall not allow any entrant to enter until
manholes/culverts is properly ventilated and safe for entry. The activity / Confined space
supervisor shall monitor the activity closely and no work shall be performed with his
supervision and monitoring by the site safety officer. “Confined space, no entry without
permit” sign board shall be posted all the times in case of no work. Straight ladder will be
used for Access and egress requirements. Pedestrian, vehicle or other barriers as necessary
and frequency of atmospheric gas testing. Types of PPE required (e.g., Dress code pant shirt /
Coverall, hardhat, safety glasses/goggles, safety shoes, appropriate gloves, protective
coveralls based on potential

skin absorption of hazardous materials), [FRC and respiratory protective equipment required
as per situation]. Work permit requirements for controlling hazards inside the confined space.
Emergency response/rescue procedures and types of equipment required for confined space
entry (e.g., rescue equipment (Tripod with manual wench and full body harness, Gas detector
and SCBA (if required), communication equipment (Mobile), whistle, air horn or manual
emergency alarm and fire extinguishers, and air movers required as per situation. Allocated
subduct shall be identified and FOC Cable shall be attached to the rope to start pulling
activities in sequence, monitor pulling activities, avoid any tension. Loose & Un reel cable near
the drum on plastic sheet. After completed cable pulling, spare cable shall be kept on Plastic
sheet and cover, which is sufficient to reach to the source, cable shall be arranged and

dressed properly inside communication room. ODTR / FOG Tester shall be used for cable
testing prior to splice to the source I DB. Perform splicing and termination activities with
proper tools. Testing activities shall be performed and ensure that cable is safe and ready to
use. Supervisor shall monitor all lifting, shifting and installation activities, and ensure all
activities carried out in proper sequence. If any precaution or changes in work procedures, he
should inform to electrical engineer or CM and ensure all crew and tools are available to
perform work safely.

3.0. HAZARD CATEGORIES RELATED TO CONFINED SPACE ENTRY


The following are some of the general categories of hazards, which may be encountered in
confined space:
3.1 ATMOSPHERIC HAZARDS
Atmospheric hazards cause the most confined-space related deaths and injuries. Atmospheric
hazards generally cannot be seen, in most cases, it is too dangerous to assume that the sense
of smell will serve as an "early warning system” air monitoring is usually required if
atmospheric hazards are suspected.
There are three general classes of atmospheric hazards:

• Oxygen deficiency
• Combustible/ flammable materials
• Toxic gases

3.1.1.0xygen Deficiency
 Humans can survive for weeks without food, days without water, but only few minutes
 without air.
 Air contains a mixture of gases but is composed primarily of nitrogen (78%) and oxygen
(21%).
 When levels of oxygen are reduced below 20%, serious health problem begins to occur
very
 quickly.
 Oxygen is consumed by a variety of chemical processes:
 Aerobic bacterial growth. (Consume oxygen to live)
 Oxidation of rusting of metals
 Combustion and displacement by other gases. (Welding and cutting torches) Effect of

3.1.2. Flammable/ Combustible Materials:


 Flammable and combustible materials are of an important concern with regards to
fires a n d explosions in c o n f i n e d s p a c e s . (Hydrogen S u l f i d e , Carbon Monoxide,
acetylene, and Methane).
 The proper mixture of fuel and oxygen varies from gas to gas, but explosion range is
defined as the area between the lower explosive limits (LEL) and the upper explosive limits
(UEL).
 When the mixture is above the upper explosive limits, it can readily move into
 Flammable range with the addition of dilution air.
 Potential source of ignition that could be found in confined spaces include open
flames, arcs from electrical equipment, hot surfaces, static electricity and frictional
sparks.
 All electrical and portable equipment will be used inside confined space as per G.l 6.012 and
SACSM Part 1-6.

3.1.3. Toxic Gas Hazards:


 Toxic or poisons gases present two kinds of risks in a confined space:
 Irritation- Many gases, existing in low concentration in the air, are irritating to the body's
respiratory and nervous system.
 Asphyxiation- Any gases which when present in high enough concentration, causes
 displacement of oxygen in the body.

3.1.4. Carbon Monoxide:


 Carbon Monoxide is one of the most common asphyxiates. It is produced by incomplete
combustion of carbon fuels.
 Carbon Monoxide kills by chemically combining w i t h t h e
 hemoglobin in red blood cells. This greatly reduces the ability of the blood to carry
 oxygen to the body tissues and brain cells.

3.1.5. Carbone monoxide Exposure Effects

3.1.6. Hydrogen Sulfide:

 This gas even more toxic than Carbon Monoxide. It is produced through the decay of
organisms and natural materials.
 This colorless gas has a characteristic rotten-egg odor at first Smell; however, after a short time
the gas renders the olfactory nerve (which controls the sense of smell) ineffective, and the
worker are no longer smells the substance, and yet it causes serious bodily harm in higher
concentrations.

3.1.7. Hydrogen Sulfide Exposure Effects

10 ppm Permissible Exposure Limit Over 8-hour shift.

50-100 ppm Mild eye and respiratory irritation.

200-300 ppm Marked increase in eye and lung irritation.

500-700 ppm Unconsciousness/death after 30-minute exposure.

1000 ppm or more Death within minutes.

4.0. MECHANICAL AND ELECTRICAL HAZARDS.


 The unexpected movement of mechanical equipment or the unexpected discharge of
electrical equipment in a confined space presents a very hazardous situation for anyone
within the space.
 Commonly encountered equipment includes: Blenders- Stirrers- mixers and agitators.

 It is very important therefore to ensure that all mechanical and electrical equipment is
locked out and tagged out when any work is to be performed in a confined space.

5.0. OTHER HAZARDS.

Entrapment, Engulfment, Collapse

 The Degree holder Civil / Mechanical Engineer / confined space and excavation competent
person is responsible to make sure manhole confined spaces are safe and secure and he
shall secure it as per SA guidelines and Sinohydro Corp. Co. Ltd. excavation plan. Site
supervisor is responsible for the safety of excavated holes/ confined space areas.
 Whensoever it is necessary to place or operate heavy equipment at a minimum of 3-meter
distance. Or Motor vehicles, cranes and heavy equipment shall be kept away from the edge
of the excavation a distance of 2 m (6.5 ft) or the depth of the excavation, whichever is
greatest.
 Hard barricades (e.g., concrete or water-filled plastic “jersey” barriers, metal fencing) shall
be used to keep motor vehicles, cranes and heavy equipment at the proper distance away
from the excavation.
 Pedestrian barricades shall be placed no less than 1 m (3 ft) from the edge of the
excavation.
 If people or vehicles could be in the vicinity of the manholes/culverts / excavation after
dark, warning lights shall be used to mark the limits of the work. If possible, the grade
should slope away from the excavation.

6.0 GENERAL SAFETY (MEANS OF ACCESS/ EGRESS. TRIPS. SLIPS FALLS. HOT. COLD.

 Exposure to higher environmental temperature during work activities.


 Exposure to corrosive substances which could cause irritation to unprotected skin.

 Exposure to rats and other pests living in the area.


 Poor or inadequate illumination.
 Exposure to noise from the use of hammers, hydraulic equipment.
 Slips and falls on wet surfaces or resulting from broken or oxidized ladder rungs.
 Means of access and egress. Straight ladders.
 Improper handling of tools and equipment can result in cuts and back injury.

7.0. PERMIT-REQUIRED CONFINED SPACE (PRCS)

 PRCS Is a space that has one or more of the following characteristics:


 Contains or could contain hazardous atmosphere.
 Contains materials that have the potential for engulfing the entrant.
 Has an internal configuration such as that an entrant could be trapped or asphyxiated?
 Any other recognized safety hazards.

The following types of facilities are designated as confined spaces:


· Manholes
· Culverts
· Drilled holes (Deeper than 1.2 m)
· Sewer lines or tunnels.
· Storage tanks
· Silos
· Underground utility vaults and storage.
· Boilers
· Pipelines.
· Pits
· Wells.
· Deep excavations

8.0. WRITTEN ENTRY PERMIT:


 Before anyone can enter the permit - required confined space (PRCS) to work, there must be a
written entry permit, which will list the following:
 Name/ location of the Permit-Required Confined Space

 Purpose for entering the PRCS.


 Date and duration of entry.
 Names of those entering.
 Names of those attending on the outside.
 Supervisor name.
 List of the expected hazard.
 Means by which you intend to isolate or control the hazards.
 Conditions under which you deem it acceptable to enter (level of oxygen, combustible, toxic
materials etc.)
 Test results of hazards in the atmosphere in the PRCS before and during the entry.
 Provisions for rescuing someone from the PRCS.
 Communication methods such as phones, radios, hand signals etc. to allow entrant and
attendants to communicate with each other.
 Equipment needed, including all the personal protective equipment, harness, alarm systems.
 Other special precautions or considerations specific to the particular space.

9.0. MONITORING FOR HAZARDS IN CONFINED SPACES


Measuring and monitoring for atmospheric hazards is one of the important things to remember
when doing confined space work.

9.1. Oxygen
According to the standard, 20% is the minimum and 23.5% is the maximum range for oxygen in
PRCS.
9.2. Combustibles
Measure the percent of LEL and according to the standard, the acceptable level for any
combustible is at or below 10% of its LEL.
9.3. Toxic
Measure the concentration of toxic substances which might be available and compare same with
TLV.

10.0. The entrant

 No one can enter a permit-required confined space (PRCS) unless he is authorized and
trained in.
 The hazards present in the space.
 Symptoms of exposure to hazards.

 How to maintain contact with the attendants on the outside.


 What personal protective equipment, retrieval lines, and harnesses required to stay
 safe in the space.
 To leave the space if he thinks he is in danger or if an alarm goes off.
 All confined space entrant should go through Sinohydro Corp. Co. Ltd. confined space entry
training prior to start the activity.
1. Review the confined space entry permit and confined space entry plan and verify that all
precautions have been properly implemented.
2. Understand the effects of exposure to potential hazardous substance(s) in the confined
space.
3. Understand the proper use of PPE that is to be used inside the confined space.
4. Not enter a confined space until they are satisfied that all necessary precautions have been
properly taken to ensure their safety, including isolation procedures, Isolation, Lockout,
and Use of Hold Tags and they clearly understand their work assignment within the space.
5. Communicate with the confined space standby man to enable the standby man to monitor
the entrants’ status (e.g., behavioral effects of hazard exposure).
6. Promptly alert the confined space standby man and other entrants, and/or exit from the
confined space as quickly as possible.

 An order to evacuate is given by the confined space standby man or the CSES.
 The entrant recognizes any warning signs or symptoms of exposure to a hazardous
condition or substance.
 An emergency alarm is activated. Monitor activities inside and outside the confined space
to determine if it is safe for entrants to enter and/or remain inside the space.
 Maintain two-way communication with entrants to monitor entrant status (e.g., behavioral
effects of hazard exposure) and alert entrants of a need to evacuate the confined space.
 Have communications equipment readily available on-site and immediately notify proper
personnel (e.g., CSES, rescue team) in event of an emergency.
 Remain at the confined space entry point until relieved by another designated standby
man or until all entrants have exited the confined space.
 Never attempt to enter the confined space, even in an emergency, until relieved.
 Perform nonentry rescues as specified by the rescue plan.
 Perform no other duties that could interfere with the primary responsibilities of a confined
space standby man.
 Order entrants to evacuate the confined space under any of the following conditions:
 An unsafe condition develops inside or outside the confined space.

 An entrant displays abnormal behavioral effects of hazard exposure. If the standby man
must leave the area and no relief confined space standby man is provided.

11.0. The Attendant

Work in PRCS will be monitored by an attendant, trained to:


Keep constant count of every person in the space confined space entry log (Attachment 03).
Keep in contact with the workers in the space.
Identify and monitor hazards.
Instruct evacuation if anything inside or outside the space looks dangerous for the workers inside.
Call for help from trained emergency response team if anyone has to be rescued from the
confined space.

12.0. ENTRY PROCEDURE & REQUIREMENT

For any PRCS after initial tests are completed and determined within acceptable limits, work may
proceed utilizing the following precautions:
At least two, and preferably three, employees will conduct all work operations in confined space.
A confined space entry permit shall be issued in accordance with GI 2.100 prior to entry into
confined spaces. Other permits may be required (e.g., hot work permit or cold work permit),
depending on the type of work to be conducted within the confined space.
Every job that requires a confined space entry shall have a designated confined space entry
supervisor (CSES) with CSE responsibilities.
Confined space entry plan shall be developed by the contractor prior to entry into a confined
space and a standby man shall be assigned by the CSES at each designated entry point and shall
continuously monitor the confined space entry while personnel are inside the confined space.
Standby men shall be provided with adequate means of communication with entrants and with
the proper personnel (e.g., CSES, rescue team) in event of an emergency.
At least one fire extinguisher shall be provided near each designated confined space entry point if
the space is within process equipment (e.g., vessel, column, tank) or where combustible or
flammable material may be present (including residue). Carbon dioxide (CO2) type fire
extinguishers shall not be used inside enclosed confined spaces.
A confined space entry checklist may be used by the CSES, etc., to help ensure safety requirements
are met.
Confined space standby men and entrants shall be physically fit.
Each designated confined space entry point shall be evaluated to ensure entrants can safely enter
and exit the confined space.

Designated entry points shall not be blocked or obstructed with equipment while entrants are

and barricades shall be posted outside the confined space to notify personnel that a confined
space entry is in progress and to prohibit entry by unauthorized personnel.

 Danger Confined Space, No Unauthorized Entry
When the potential exists for persons or objects to fall into a confined space, additional warning
signs and/or barricades shall be provided.
Prior to entry, chemical, and physical hazards shall be eliminated or controlled
Mechanical ventilation (e.g., air movers) shall be used to ensure removal of all hazardous airborne
contaminants in confined spaces where a hazardous atmosphere exists
Atmospheric gas testing shall be in accordance with GI 2.709
The proper personal protective equipment (PPE) shall be provided to personnel entering a confined
space and to each standby man. PPE shall be continuously used during the confined space entry.
Only properly trained and authorized personnel shall enter a confined space.
Personnel entering/exiting a confined space shall sign a log in/out sheet.
Ignition sources shall be eliminated or controlled within a confined space. If ignition sources are
present (e.g., Lighting shall be provided as needed to safely enter, exit, and work inside a confined
space.
Internal combustion engines shall not be positioned inside a confined space or at a location that
could allow exhaust to enter the space.
To facilitate nonentry rescue, retrieval systems or methods shall be used whenever an entrant
enters an enclosed confined space that requires vertical entry unless the retrieval equipment
would increase the overall risk of entry or would not contribute to the rescue of the entrant.
A retrieval system is the equipment (including a retrieval line, full-body harness, and a lifting
device) used for nonentry rescue of persons from vertical confined spaces.
Each entrant shall wear a full-body harness, with a retrieval line attached at the center of the
entrants back near shoulder level or above the entrant’s head.
The other end of the retrieval line shall be attached to a mechanical device (e.g., winch attached to
a tripod) so that rescue can begin as soon as the confined space standby man becomes aware that
rescue is necessary.

The retrieval system shall be tested at the beginning of the entry into the vertical confined space to
ensure that the system is properly setup and functioning. If the system does not work all entry
activities must stop and all entrants must exit the space until the retrieval system is operational.
The CSES shall arrange these breaks accordingly.
Flammable/combustible materials shall not be stored inside a confined space.
In the confined space atmosphere shall be considered immediately dangerous to life or health
(IDLH) and positive-pressure self-contained breathing apparatus (SCBA) or full-face, positive-
pressure type air-line respiratory protection equipment with an emergency escape cylinder shall be
used by all personnel entering the confined space. See Chapter I-3, Personal Protective Equipment
(PPE).
If the confined space entry activity is suspended, the entry point(s) shall be barricaded and a “NO
ENTRY” sign shall be posted.
When work is complete and the confined space is ready to be returned to normal service, the
confined space shall be inspected to ensure all equipment and tools have been removed, etc.

13.0. Gas tester


He shall be on site to monitor all confined space environment situations and third party trained
gas tester will conduct gas test with valid calibrated gas testing monitor and log will be
maintained. He must keep his valid 3rd party certification with him to be shown as demanded by
the Aramco / SAMPT.

14.0. Confined space supervisor


CSE Supervisor & Standby man shall be trained by HSE department to perform the job.
Confined space supervisor is responsible for the performance of all confined space activities safely.
Excavations that have been subject to runoff because of rain etc. shall be inspected before use by
the excavation competent person in accordance.
Confined space supervisor and standby man is required prior to start the activity inside confined
space in accordance with SA CSM I,6 Confined space, below requirements.

15.0. Confined Space Attendant / Confined space standby personnel

A person trained in rescue procedure and ERP; employee shall remain outside of the space to be
available in case of an emergency.
This individual shall be in direct communication with the other workers in the confined space, and
have the capability to reach emergency response action team (ERAT) using emergency contact
number (Attachment 04).

Continuous monitoring of the atmosphere inside the confined space, and at any time the
monitoring devices indicate the presence of toxic or flammable gases, or a decreased level of
oxygen, employees are to evacuate the space at once.
Review the confined space entry plan and applicable work permits to understand the confined
space hazards, precautions, responsibilities and emergency procedures.
Never attempt to enter the confined space, even in an emergency, until relieved.
Perform nonentry rescues as specified by the rescue plan.
Perform no other duties that could interfere with the primary responsibilities of a confined space
standby man.
Understand the effects of exposure to potential hazardous substance(s) in the confined space.
Maintain a confined space entry log and maintain a continuous count of entrants.
Prevent unauthorized personnel from entering the confined space.
Have communications equipment readily available on-site and immediately notify proper
personnel (e.g., CSES, rescue team) in event of an emergency.
Monitor activities inside and outside the confined space to determine if it is safe for entrants to
enter and/or remain inside the space.
Maintain two-way communication with entrants to monitor entrant status (e.g., behavioral effects
of hazard exposure) and alert entrants of a need to evacuate the confined space.
Remain at the confined space entry point until relieved by another designated standby man or
until all entrants have exited the confined space.
Order entrants to evacuate the confined space under any of the following conditions:
An unsafe condition develops inside or outside the confined space.
An entrant displays abnormal behavioral effects of hazard exposure.
If the standby man must leave the area and no relief confined space standby man is provided.

16.0. EMERGENCY AND RESCUE PROCEDURES


If someone has to be rescued from a confined space, that is the job of the rescue team. The rescue
team will Know how to:
Close off the area
Use SCBA, CPR, and other rescue and first-aid techniques and equipment.
Confined space entrant shall wear harness while entering in the confined space. Standby man/
helper will hold the rope line which connected with harness of entrant to communicate.

Rescuer will enter the confined space by ladder (wearing a SCBA only if required).
Rescuer will get the effected into the stretcher and connect the sling with the Tripod wench hook
