I have the honor to apply for permit to solicit, receive contribution from the public for “Charitable
or Public Welfare Purpose” pursuant to the provision of Act No. 4075, pertinent information and
data about the committee or organization, which I represent, are furnished below for your
information, to wit.

3. Specific aims or purpose for which the permit is sought: SOUND SYSTEM INSATALLATION

4. Method to be employed: POPULARITY CONTEST

(Only lawful means, ways or service are acceptable, such as voluntary contribution, benefits affairs,
social boxes, beauty queen and popularity contest and benefits affairs such as declamation, essay
contests)
(Lottery, raffles, card game, Bingo, Canastra and other games of chance or hazard are prohibited to
Article 195 of the Revised Penal Code.)

5. Approximately amount as goal needed to be raised during the drive: P60, 000.00

6. Period and area to be covered by the fund campaign: January 2024 to February 2024
within the area of Luzon, Visayas and Mindanao.

In filling this application, I have fully observed the provisions of Administrative Order No. 206,
series of 1962, implementing Act No. 4075 and the fundamental requirements prescribed at the
back thereof. I will surrender the original Solicitation Permit immediately after the termination
period JO the fund drive and within one month I will submit duly certified report consisting of an
itemized statement or collection of disbursement.

“WHEREAS, the Busao National High School of Busao, Maribojoc, Bohol thru the
School Parents and Teachers Association Board of Directors (SPTA BOD) has expressed their
intent and desire to support the financial needs of the school for the purchase and installation of
school sound system;

“WHEREAS, the estimated amount for the said project is about P60, 000.00 and the
association has no funds to finance the above-mentioned project;

“WHEREAS, upon motion of Miss Marife Igpit to hold a Popularity Contest (King and
Queen) means of fund raising and duly supported by all SPTA BOD Officers and members.

“RESOLVED, as it is hereby resolved to hold a popularity contest and to request Hon.

Romulo A. Manuta, Municipal Mayor, through the Department of Social Welfare and
Development of Maribojoc. Bohol to grant the association a permit for fund raising in the form of
Envelope System from January 2024 – February 2024.

“RESOLVED FINALLY, to authorize the Officer-in-Charge Mrs. Myrna Hidlao to sign the
certification and pertinent papers relative to the same.

WHEREAS, the Busao National High School – School Parents and Teachers
Association Board of Directors (SPTA BOD) resolved to purchase and install a school sound
system.

RESOLVED, as it is hereby resolved that each and everyone of the officers pledge to
assume responsibility for all donations received, that they will limit the expenses incident to the
holding of the fund drive not more than 10% for the operational expenses and the balance of
90% will be appropriated for the said project, that they do not get shares in the net proceeds to
be derived there from; and that lawful means shall be employed during the drive.

RESOLVED FINALLY, to authorize the Officer-in-Charge of Busao National High School

Mrs. Myrna D. Hidlao to be the lawful representative of the SPTA duly authorized to sign the
application form for the solicitation permit with the Department of Social Welfare and
Development.

Presiding Officer’s Time

The body discussed and planned for crafting of the School SIP projects for the school
year, and their priority is the installation of the school sound system. It has been agreed by the
majority to conduct a fund raising campaign and request the Honorable Mayor Romulo A.
Manuta, through a resolution, to grant permit to the association related to the fund raising
activities, in the form of popularity contest (King and Queen). The said fund raising contest and
collection will start on January 2024 to February 2024 as last canvassing and no other matter
has been discussed or delivered on motion by Mrs. Rosario Dapiton and duly seconded by Mrs.
Teresita Napiňas, the meeting ended at 12:05 noon.

Article I: Name and Domicile

Section 1: - That the association shall be called Busao National High School – School
Parents
and Teachers Association (BNHS – SPTA)

Section 2: - That the place of the association is situated in Busao, Maribojoc, Bohol

Article II: Objectives and Purpose

Section 1: - To engage on any lawful income generating activity in order to raise funds for
school improvement particularly the installation of a school sound system.

Section 2: - The association shall aim to make all members useful not only to themselves
but
towards the society as well.

Article III: Area of Operation

Section 1: - The area of the operation shall be within Luzon, Visayas and Mindanao,
wherein
the friends, relatives or acquaintances of the candidates’ parents are residing.

Article IV: Financial Matters

Section 1: - The association may collect fees and may raise funds as the need arises
specifically for school improvement.

Article V: Officers of the Association

Section 1: - The officers of the association are the following:

Section 2: - That this constitution and By-Laws was ratified on September 29, 2023 at
