CONCEPT PAPER
A. Description of the Activity
TITLE: “Nursing with a Heart: A Gift-Giving Program for the Grade 5 & 6 IP pupils of
Kasaysayan Elementary School”

Target Group: 15 Grade 5 & 6 IP pupils of Kasaysayan Elementary School

Date of Implementation: January 27, 2024 / 10:30 am to 12:00 nn

“Truly, with love, nothing is impossible.”

As the Christmas season rolled out, each of us still felt the breeze of love, kindness, and
generosity that has swept through. As everybody is slowly returning to the normal daily
activities and anticipating the next advent season there are also whom that didn’t felt the spirit
of the past advent season. This includes the Dumagat tribe of Sitio Kasaysayan, Diagyan,
Dilasag, Aurora. During this season rainy season also starts in the municipality isolating the
sitio from the rest of the municipality because of flooding making the river that bounds the sitio
not passable to humans and any kinds of vehicles. So, for this activity I would like to surprise
their children who are enrolled at Kasaysayan Elementary School as Grade 5 & 6 through a
gift giving program by looking for other support partners. I truly believe that alternative gift-
giving activities, such as donation of hygiene kits, gift fairs, donation of slippers and toy
exchanges offer youth and children a way to give back to their communities while learning
valuable Christian Spirit and Social Involvement skills. This outreach activity is aligned with the
Catholic Education Association of the Philippines CEAP-JEEPGY advocacies and the CICM
Thrust. Specifically, through transformative health education grounded on Engaged
Citizenship, Gender Sensitivity and Youth Empowerment.

The above-mentioned activity and program are aligned with the UN Sustainable Development
Goals (SDG’s) and CICM Advocacies. Specifically, through dealing with specific vulnerable
conditions and diseases and supporting their community (SDG 3, Good Health and Well-
Being). This community outreach program is sustainable way of introducing the importance of
health such as infection prevention and control and self-care practices based on the
collaboration between healthcare workers such as Hospital Nurses, youth, vulnerable children
and other partners taking the form of a knowledge bridge. The conduct of community services
is about building rapport, relationship, and linkages to the community. Since (1) love of self,
neighbor and family, (2) health and ecological consciousness and (3) peace and global
solidarity are among the core values of University of Perpetual Help System Dalta, it is very
significant that the neighboring community with vulnerable population shall benefit from the
community outreach programs conducted by the school. Finally, every program such as this
proposed activity shall be evaluated immediately to find out its strengths and weaknesses and
to address all the problems encountered before, during, and after the outreach using the
impact evaluation approach.
