ACCOMPLISMENT REPORT

GRADE 9 LEVEL
S.Y. 2021-2022

The following are the activities accomplished by the Grade 9 level in


the school year 2021-2022.
A. Module Preparation, Distribution and Retrieval
B. Meetings (Orientation, PTC, Grade Level Meetings)
C. Home Visitations (for at risks students)
D. Virtual Graduation
E. LAC

A. Module Preparation, Distribution and Retrieval


Since there was no face-to-face learning, students of Grade 10 level
chose to have Modular learning Modality. With this, teachers are
expected to prepare the module good for two weeks. Preparation of
module entails a lot of printing and photocopying since the module
given is not enough. Teachers must provide more copies so each
student can receive their own module. Part of the preparation of the
module is for the advisers to collect the module of all the subjects and
place it in one plastic and one envelope. It was done this way so it
would be easy for the barangay officials to distribute it. The student’s
module was then placed in the box for its specific barangay complete
with labels of name, contact number and address. Each barangay was
helping the school in the distribution of the module. Most of the
barangay collected the module in the school every Friday, as they
collect the module, they also return those that was already answered
by the students. For those who requested to have their module get
and returned to the school they were given specific schedule.
B. Meetings (Orientation, PTC, Grade Level Meetings)
To talk about the problems within the grade levels and the
strategies that should be employed in the preparation, distribution
and retrieval of modules, the grade level conducted meetings. They
should also conduct deliberations on students’ progress.
Also, there were meetings with parents to ensure that they are
aware of the progress of their students. Those who failed to submit
their modules were also called so the teachers can help them finish
their modules.

C. DULOG (Home Visitation)

For those parents who failed to come to school despite the constant
reminder of the advisers were visited at home with the help of their
barangay officials. The advisers listed everyone in a specific barangay
with problems on submission and visited their home to talk about the
problems they have encountered during the implementation of the
modular distance learning.

Advisers with the help of the chairperson talked to the parents


and agreed that students will work on their module and submit to
their respective barangay during the schedule of submission.

D. Virtual Moving Up

Despite the pandemic the grade level were able to conduct


virtual moving up ceremony last July 8, 2022, via Stream yard at 1-3
pm. (see attached documents for virtual moving up). The theme in the
virtual moving up was “Gradweyt ng K to 12: Masigasig sa mga
Pangarap at Matatag sa mga Pagsubok”. The virtual ceremony
lasted for two hours.
