MEETING REPORT

END RESULTS:
The meeting started with a prayer led by Mrs. Maria Flor Jorquia. After the prayer our
school head Ma’am Bevelyn Paler presented the school monitoring tools.
Our school head clarifies on what are should be put on the students’ issues and
concerns of based on the modules they have receive by subjects. If there is no issues and
concerns on certain subject, the subject teacher must justify it by putting remarks on his/her
reports. An example of common issues face is the online copyahan or the presence of answer
keys of module online, this can be used on the issues and concerns of the students.
Next, we talk about the contextualize materials or list of activities to conduct in relation
to contextualization by subjects. The activities that are not present should be contextualized
based on the present materials or available in their community. For example, the contextualize
activities in science of Ms. Jacqueline Suson where she asked her students to make a xylophone
using bamboo instead of buying a xylophone, the student made their own xylophone using the
available materials in the community. In making the contextualize report the name of activity
and MELC’s should be put so that it can be determined what subject area it is belong. In the
subjects handled must locate what are the possible activities that can be used to make a
contextualize report.
For the printing of modules, it is implementation that the printing of modules will be
reduce to 50% printing of modules for second quarter due to overuse of printers. Class adviser
should divide their students into two sets set A and B where they will undergo shifting in
receiving of modules. Class adviser must post the list of learners for set A and B outside their
classroom. Only Grade 11 and 12 HUMMS will not follow shifting of modules because of least
number of students.
Another concern tackled is all about the incomplete modules in DepEd portal, only
senior high and grade 9 TLE are allowed to make a LAS to those subjects that are not available
in the portal. The grade 9 English and MAPEH don’t have available modules on portal. This
concern is still on hold and will be presented to our DIC.
We also talked about on the requisition and issued form, it was turn over to Mrs.
Jorquia (Property Custodian) due to the Supply Officer (Mrs, Anguiled) is pregnant. All teachers
that requested materials should be written in the requisition form and all materials that was
used on the month of September should be listed so that the supply officer will be able to track
all the materials that was used for this school year. In requisition form the school supplies and
hardware materials are separated upon request. Each teacher was given a copy of requisition
form, this form that will be used when a teacher would like to request any materials needed for
his/her classroom. A separate logbook is also present to the property custodian for another
copy of. All items requested will only release by the property custodian.
Lastly Ma’am Bevelyn Paler asked us on the updated tracking of printed of modules we
should entry an updated tracking of printed modules on the site of tracking of printed module
and pass a hard copy on our school head.

REQUIRED INPUTS:
All teachers must submit an issues and concerns report for the first quarter.
All teachers also must submit at least one contextualize report for the upcoming school
monitoring.
Teachers should follow the implementation of 50% printing of modules for the second
quarter.
Print out of list of learners for shifting schedule must be posted outside the classroom by
the adviser.
All teachers must use the requisition form and submitted to the property custodian when
there is any request any materials needed.
All teachers must submit a hard copy of updated tracking of modules to the school head
and also an input online for updated tracking of modules.

COMMUNITY ON PRACTICE REPORT

PURPOSE:
a. Monitoring and Evaluation
b. QATAME TOOLS/ Customer Feedback Form

END RESULTS:
After our LAC Session, we proceed to have our community of practice for monitoring
and evaluation headed by Mrs. Jacqueline Dacudao.
Monitoring is a repeat process of data collection can be weekly, quarterly or annually to
measured weather the program is doing what it is set out to do. While monitoring is a process
that systematically and objectively assess all the elements of a program to determined its
overall worth or significance.
A customer feedback form was given to the advisers for the evaluation on the
parents/guardians’ feedback on module distribution and retrieval. In customer feedback form
where we can determine the problems in doing the module distribution and retrieval. After the
parents/guardians answers the customer feedback it will undergo consolidation on all the
answers to get the result. All consolidated results of the adviser will be also consolidated so
that we will be able to get the overall result of customer feedback for the first quarter. If the
ratings are low, the technical assistance team will suggest or give recommendations on how to
improve the ratings on next customer feedback for module distribution and retrieval.
After the presentation of different monitoring tools for monitoring and evaluation, the
selection of QATAME (Quality and Assurance, Technical and Assistance and Monitoring and
Evaluation) Team was selected. For Quality and Assurance Team Mrs. Emilyn Anguiled is
chosen as the head of team and its members are Mr. Rolly Romeo, Ms. Kemberly Matulac and
Ms. Jinky Glen Montejo. In Technical and Assistance Team, Mr. Ruel Tario was the head and
Mrs. Marylou Elmidor, Mrs. Maria Flor Jorquia and Ms. Jacquilyn Suson are the members. Lastly
the Monitoring and Evaluation Team are Mrs. Jacqueline Dacudao as the head and its members
are, Mrs. Rogelyn Junsay, and Mr. Franknur Valeriano and Mrs. Russel Palmero.
The monitoring tools for project plan for each department was also discuss where each
department must submit a target project plan for this school year, in order to come up a
project for this school. This project plan will be proposed so that it will be included in the
budget of school projects. A community on practice is required to each department to come up
and agreed on the project of the department.

REQUIRED INPUTS:
All adviser must let at least 50% of students’ parents/guardians population to answer the
customer feedback for the evaluation for module distribution and retrieval.
After the all the parents/guardians answers the customer feedback form the result will be
consolidated to gained the result.
All department must conduct a community on practice to come up a project plan for this
school year.
All department coordinator must submit a proposed project plan.

PURPOSE:
a. SF2 New Format

END RESULTS:
Mr. Ruel Tario explained to us the new format for School Form 2, since the School
Format that we have use are not already updated. The new format of school format 2 contains
LACT (Learners Attendance Conversion Tool) 1, 2, and 3. LACT 1 is used when the mode of
curriculum used is online, LACT 2 for distance learning and for LACT 3 is for printed module.
Since when are accommodating printed module so we are going to use LACT 3. Also the other
changes for new format of school form 2 are on the late enrolled students which are enrolled
on September 17, 2021 and beyond the percentage of total students should be more that
100%.
REQUIRED INPUTS:
All the advisers must submit a new format of school format 2 for the month of
November and also changing the record on the month of October and will be submitted
together with the month November.

COMMUNITY ON PRACTICE REPORT

PURPOSE:
a. DepEd Order no. 14, s. 2021

END RESULTS:
For the clarification on the new format of school forms, Mr. Ruel Tario oriented us
on the Deped Order no. 14 series 2021 or the Interim Guidelines on the Preparation,
Submission and Checking of School Forms. He presented us the several school forms that are
to be prepared for the school year 2021-2022. School Forms 1, 2 and 9 will be prepared by the
advisers and the rest are prepared by assigned teachers. Of all the school forms presented only
the school form 2 has a new format. The new format of school form 2 contains LACT (Learners
Attendance Conversion Tool) 1,2 and 3. We are asked to use the LACT 3 since we are using
the printed module as method of learning. The other changes on the school form 2 is all about
the computation of the attendance of the students. In able to generate the attendance of
students it should be based on the number of school days, number of required modules to be
submitted and number of modules that was submitted. Number of days (6 days including
Saturday) is divided on number of submitted modules of the learner then multiply to the required
modules to be submitted to generate the attendance of the students. If the result of the
attendance is 100% it should be justified on remarks for clarification. Also students that are
transferred in for this school year must be address on side on their originate school.
Lastly a follow up on the submission of school form 2 for the month of November must
be submitted before November 11, 2021 together with the change format of school form 2 for
the month of October.

November and also changing the record on the month of October and will be submitted
together with the month November.
