HSE Alert # 4 (2019) Dropped Object Prevention

An incident occurred on one of the HPD Projects involving a dropped


object. A steel tie-rod plate weighing approximately 300g fell 7m from
a scaffolding platform and struck a worker below on his hard hat whilst
he was carrying out housekeeping duties. The Worker sustained minor
injuries to his head and given 2 days sick leave. This incident was
classified as a Medical Treatment Case.

Dropped Object - Root Cause

 Incompatible activity (line of fire) - Two activities carried out at the same
time in different level.
 No exclusion zone and warning sign to warn personal from entering
underneath the activity area.
 Side netting and toe-boards on scaffolds were missing.
 Improper storage / poor housekeeping on the scaffolding platforms.
 Risks were not identified and assess potential problem areas.
 Scaffolding modified after the inspection without authorization and not
followed re-inspection process.
