A RESOLUTION AUTHORIZING SK CHAIRMAN JOHN DERICK A. RASOS AND SK

TREASURER JOEBERT M. TAMANG TO OPEN AN ACCOUNT AT LAND BANK OF
THE PHILIPPINES CABAGAN BRANCH, CABAGAN, ISABELA AND AS
SIGNATORIES OF CHECKS OF THE SK OF BARANGAY BALELLENG, SANTO
TOMAS, ISABELA.

WHEREAS, the Elected SK Chairman of Balelleng, Santo Tomas, Isabela JOHN DERICK A.
RASOS.

WHEREAS, the appointed of SK Treasurer concurred by the Members of the Sangguniang

Kabataan of Barangay Balelleng, Santo Tomas, Isabela is JOEBERT M. TAMANG

WHEREAS, they are authorized as the signatories of checks and any transaction of the SK
Balelleng, Santo Tomas, Isabela;

NOW THEREFORE, on motion of all Members of the Sangguniang Kabataan present;

RESOLVED, as it is hereby resolved to authorized SK Chairman JOHN DERICK A. RASOS

and SK Treasurer JOEBERT M. TAMANG to open an account of the SK of Barangay
Balelleng, Santo Tomas, Isabela at the Land Bank of the Philippines, Cabagan Branch, Cabagan,
Isabela, and as signatories of checks of SK in Barangay Balelleng, Santo Tomas, Isabela.

RESOLVED FURTHER, that copies of this resolution be forwarded to the Land bank of the
Philippines for appropriate action.

revoked or suspended
